Zusammenfassung
Unsere Versuche zeigen, daß das Stratum synoviale eine gute Resorptionsfähigkeit besitzt. Eine intraartikulär injizierte wäßrige Lösung wird innerhalb von 20–25 min in die Blutbahn aufgenommen. Durchblutung und Resorption unterliegen den gleichen vegetativ nervösen Regulationen wie in anderen Körpergebieten. Sie können durch eine Entzündung an gelenkfernen Gebieten, reflektorisch durch eine Entzündung der Haut über dem Gelenk, durch örtliche oder intravenöse Injektion von Adrenalin, durch eine Barbituratnarkose und Muskelrelaxantien im Sinne einer Verzögerung beeinflußt werden. Cortison hat am gesunden Stratum synoviale keinen Einfluß auf die Resorption.
Jede Entzündung des Stratum synoviale führt über eine Irritation des Endstrombahngebietes zu einer Resorptionsverzögerung, wobei diese von Art und Grad der Entzündung abhängig ist.
Bei einem pH von 4,6 und bei Anwendung eines bestimmten Druckes wird die Resorption durch Hyaluronidase über eine verbesserte Diffusion zum Kapillar-system anfänglich gesteigert. Eine sekundär durch Hyaluronidase ausgelöste Entzündung jedoch, die sich in Form von Ödem, Hyperämie, petechialen Blutungen und mäßiger granulozytärer Infiltration manifestiert, verzögert die Resorption, wobei sich aber mit dem Abheilen der Entzündung wieder normale Werte einstellen.
Bei einer durch Formalin erzeugten Arthritis findet man eine oberflächliche Koagulationsnekrose der Synovialzellschicht und der in ihr liegenden Gefäße, die vom subsynovialen Gewebe langsam durch Organisation resorbiert wird. In allen Stadien dieser Entzündung, beobachtet vom 1.–12. Tage nach der Injektion, findet man eine beträchtliche, etwa in gleicher Höhe bestehenbleibende Resorptionsverzögerung.
Im Vordergrund der allergisch-hyperergischen Arthritis steht eine Gefäßalteration, die 24 Std nach Auslösung der Antigen-Antikörperreaktion zu einer Resorptionssperre führt. Zu diesem Zeitpunkt findet man bei einer hochgradigen, ödematösen Auflockerung des Synovialgewebes Gefäßwandnekrosen und eine massive granulozytäre Infiltration. Die Synovialzellschicht ist völlig abgeschilfert. Mit dem Abheilen der Entzündung nähern sich auch die Resorptionswerte wieder der Norm.
Aus den Versuchen wird der Schluß gezogen, daß im Mittelpunkt einer „Gelenkerkrankung“ eine Alteration des Stratum synoviale stehen muß, die über eine Änderung der Sekretion bzw. Resorption zu Funktionsstörungen im Gelenk führt.
