Zusammenfassung
Zur Analyse der O2-Diffusionsverhältnisse in der Lunge wurden Messungen des alveolären Gaswechsels im hypoxischen Bereich an 18 isolierten, von Spendertieren durchströmten Lungenlappen des Hundes durchgeführt. In 5 Versuchen wurde die Erythrocytenkonzentration, in 13 Versuchen die Durchblutung variiert, wobei der venöse und der alveoläre O2-Druck weitgehend konstant gehalten wurden.

Bei wechselweiser Durchströmung des Lungenlappens von Spendertieren mit hoher und niedriger Erythrocytenkonzentration blieb die alveolär-arterielle O2-Druckdifferenz ({ie211-1}) unverändert, der danach berechnete O2-Diffusionsfaktor ({ie211-2}) war der Erythrocytenkonzentration angenähert proportional.

Auch bei Veränderungen der Durchblutung änderte sich die {ie211-3} nicht, während der danach berechnete {ie211-4} der Durchblutung proportional war.
Für diese Befunde ergeben sich zwei Deutungsmöglichkeiten: a) Die Zunahme des {ie211-5} mit der Durchblutung war reell. Dann war — auf Grund des Verhaltens bei Veränderung der Erythrocytenkonzentration — der Hauptanteil des O2-Diffusionswiderstandes in den Erythrocyten zu suchen. b) Die gemessenen {ie211-6} waren nicht durch eine Diffusionsbegrenzung, sondern durch eine „Verteilungsstörung“ bedingt. In diesem Falle hat man nur „scheinbare“ {ie211-7}-Werte berechnet: der wahre Wert für {ie211-8} mußte viel größer sein. Diese Erklärung wird als die wahrscheinlichere angeinlichere angesehen.
Summary
The aim of this study is to investigate the alveolar-capillary diffusion of O2, especially to determine if the major part of the resistance to O2 diffusion is located in an “alveolar membrane” or in the erythrocytes. For this purpose measurements of alveolar gas exchange in the hypoxic region were performed on 18 isolated lung lobes of the dog, perfused with blood from donor animals. In 5 experiments the red cell concentration of the perfusion blood was varied, in 13 experiments the perfusion rate was changed. In all experiments the venous and the alveolar O2 pressures were kept nearly constant. For technical reasons in most experiments such venous and alveolar O2 pressures were chosen as to result in an elimination of O2 by the lung lobe.

By variation of the erythrocyte concentration in the perfusion blood (on the average by a factor of 3,2) the alveolar-arterial O2 pressure difference was not changed, whereas the calculated O2 diffusing capacity was nearly proportional to the erythrocyte concentration.

Changes in the perfusion rate (on the average by a factor of 2,7) did not alter the alveolar-arterial O2 pressure difference, while the resulting O2 diffusing capacity turned out to be very nearly proportional to the perfusion rate. (The existence of an alveolar-arterial CO2 pressure difference could not be conclusively demonstrated in these experiments.)
Two possible interpretations of these results are considered:

a)
The increase in the O2 diffusing capacity with the perfusion was real. In this case the major part of the resistance to O2 diffusion had to be located in the erythrocytes, because of the results with changed erythrocyte concentration.

b)
The measured alveolar-arterial O2 pressure differences were not created by a homogeneously distributed barrier to O2 equilibration, but by an unequal distribution of ventilation, perfusion or diffusion parameters. In this case the calculated O2 diffusing capacity represented an “apparent” O2 diffusing capacity only: the true value of the O2 diffusing capacity could have been much higher. This second interpretation is considered as the more probable one. It is attempted to estimate the kind and the extent of the unequal distribution necessary for explanation of the measured data.
