Summary
Standardized hemorrhagic shock is induced in nine anaesthetized dogs at a blood pressure of 40 mm Hg. 200–300 mg CaCl2 or 0,25 mg Strophanthine are injected i.v. during early oligaemic shock, while the test animals are still giving blood into the reservoir at fixed hypotonia. These injections cause an increase of the extracorporal blood volume or rather of the arterial pressure, if the connection with the reservoir is shut. These reactions can be attributed to an improved heart contractility, which formerly had been lessened by hypotonia. If these drugs are injected before blood withdrawal, they do not have any effect on blood pressure.
In a second test group the stage of hypotonia at 40 mm Hg is interrupted after 20 minutes and blood is reinfused until the average arterial pressure has increased to 90 mm Hg. The same test injections of CaCl2 and heart glycosides even then cause clear pressure increases. If normal pressure is maintained for 2 to 3 hours, new injections of the cardiaca do not show any effect. Myocardial failure has been repaired.
Zusammenfassung
An neun narkotisierten Hunden wird ein standardisierter hämorrhagischer Schock bei einem Blutdruck von 40 mm Hg durchgeführt. In der frühen oligämischen Phase — wenn die Versuchstiere bei fixierter Hypotonie noch Blut in das Vorratsgefäß abgeben — werden 200–300 mg CaCl2 oder 0,25 mg Strophanthin i.v. injiziert. Als Folge der Injektionen nehmen die extracorporale Blutmenge, beziehungsweise bei unterbrochener Verbindung zum Vorratsgefäß der arterielle Blutdruck, zu. Diese Reaktionen werden auf eine Besserung einer durch die Hypotonie bedingten Kontraktilitätsschädigung des Herzens zurückgeführt. Werden die Substanzen vor dem Blutentzug injiziert, beeinflussen sie den Blutdruck praktisch nicht.
In einer weiteren Versuchsgruppe wird das Stadium der Hypotonie von 40 mm Hg nach 20 min abgebrochen und soviel Blut reinfundiert, bis der arterielle Mitteldruck auf 90 mm Hg gestiegen ist. Die gleichen Testinjektionen von CaCl2 und Herzglykosiden verursachen auch jetzt deutliche Blutdruckanstiege. Werden die Hunde dann 2–3 Std bei Normaldruck belassen, sind erneute Injektionen der Cardiaca wirkungslos. Die Myokardschädigung hat sich repariert.
