Summary
The clinical and morphological effects of a non-operatively corrected, traumatic carotid-jugular fistula are reported and discussed. As a result of a neck injury the patient, 58 at time of death, developed a traumatic aneurysm of the common carotid artery with signs and symptoms of stasis of the neck and facial regions, an etiologically unexplained visual defect on the contralateral side of the fistula, and finally, cerebral complications which caused death. Neuropathological investigation showed that the long-standing hemodynamic peculiarities of the shunt had led to a severe ectasia of the cerebral vasculature and of the contralateral internal carotid artery. This was accompanied by chronically recurrent blood-brain barrier disturbances, edema, and the beginnings of tissue damage. In addition, the ectasia of the contralateral carotid had led to a pressure atrophy of the adjacent optic nerve. Apart from the often described effects of an a-v shunt on the cardio-vascular system, an additional cerebral complication is described here and verified patho-anatomically.
Zusammenfassung
Bericht und Diskussion über die klinischen und morphologischen Auswirkungen einer nicht operativ korrigierten, traumatischen Fistel zwischen A. carotis communis und V. jugularis interna. Bei dem beim Tode 58 Jahre alten Patienten entwickelten sich im Anschluß an eine Splitterverlezung der li. Halsseite ein traumatisches Aneurysma der A. carotis communis mit Stauungserscheinungen im Hals- und Gesichtsbereich, ätiologisch unklare Sehstörungen auf der contralateralen Seite der Fistel und schließlich cerebrale Komplikationen, an denen der Patient verstarb. Die eingehende neuropathologische Untersuchung konnte zeigen, daß die über Jahrzehnte bestehenden hämodynamischen Besonderheiten des Shunts zu einer hochgradigen Ektasie der Hirngefäße und der contralateralen A. carotis interna geführt hatten, welche von chronisch rezidivierenden Schrankenstörungen, Ödemen und beginnendem Gewebsuntergang begleitet war. Die Ektasie der contralateralen Carotis hatte darüber hinaus zu einer druckbedingten Atrophie des adherenten N. opticus geführt. Neben den in der Literatur häufiger beschriebenen Auswirkungen eines a-v-Shunts auf Herz und Kreislauf werden im vorliegenden Fall zusätzlich die cerebralen Komplikationen beschrieben und patho-anatomisch belegt.
