Summary
In animal experiments Costall and Naylor (1977) showed that Tiaprid, a benzamide derivative of the orthopamids, inhibits dopamine receptors in the caudate nucleus and putamen but not in the mesolimbic system. Involuntary movements in patients with extrapyramidal movement disorders were reduced by 300 mg daily. Good results were obtained in 56% of 93 patients (25 in this study and 68 from the literature). On the whole the efficacy of Tiaprid seems far better than that of neuroleptics, and side effects are definitely fewer.
Zusammenfassung
Tiaprid, ein Benzamid-Derivat aus der Reihe der Orthopramide, inhibiert nach tierexperimentellen Befunden (Costall and Naylor, 1977) relativ spezifisch Dopaminreceptoren von Caudatum und Putamen, ohne daß die Dopaminreceptoren im mesolimbischen System wesentlich beeinflußt werden. Im klinischen Versuch mit 3×100 mg Tiaprid pro Tag konnten extrapyramidale Bewegungsstörungen bei etwa der Hälfte der untersuchten 25 Patienten deutlich gebessert werden. In Verbindung mit Untersuchungsergebnissen anderer Autoren (insgesamt 93 Patienten) ist ein guter Erfolg bei 56% der Patienten zu verzeichnen. Dabei sprechen Choreoathetosen und Hyperkinesen, insbesondere im Gesichtsbereich, und zwar unabhängig vom Entstehungsmodus günstiger an als Torticollis und Torsionsdystonien. Da bisherige Therapieversuche mit Neuroleptika weniger günstige Ergebnisse und insgesamt mehr Nebenwirkungen aufweisen, stellt Tiaprid eine Bereicherung für die medikamentose Behandlung extrapyramidaler Bewegungsstörungen dar.
