Summary

We have brought together a few less familiar disorders occuring in the area of the carotid siphon, so that against this background the diagnostic features of meningiomas of the lesser wing might be more clearly defined. — Condensation of the lesser wing is commonly seen with meningiomas. A vascular blush should be considered as a sine qua non in this respect. Complete tomographic examination including anaxial view of the sphenoid bone is recommended. In opposition to a recently published view, we wish to stress that arteriopathic processes may be localized to the infraclinoid and even precavernous portions of the internal carotid artery. A rete mirabile type opacification as obtained in aplasia or hypoplasia of the carotid siphon should be clearly distinguished from a tumoural vascular blush. — Spontaneous epidural haematomas of the temporal fossa, arteriopathic processes and congenital aplasia or hypoplasia of the carotid siphon must now be included in the differential diagnosis of meningiomas of the lesser wing.

Résumé

Nous avons rassemblé quelques cas de pathologie inhabituelle au niveau de la région du siphon carotidien. Même si sa condensation représente un facteur commun dans nos trois cas de meningiome la tâche vasculaire tumorale demeure la base de ce diagnostic. Un examen tomographique incluant en particulier des clichés en projection axiale du sphénoide nous est donc apparu quand même essentiel. — Nous avons ré-insisté sur le fait qu'à l'encontre d' opinions récentes, certaines artériopathies peuvent être localisées au niveau de la portion précaverneuse et infra-clinoidienne de la carotide interne. Par ailleurs nous avons souligné l'importance de bien distinguer une opacification d'un rete mirabile obtenu dans les agénésies du siphon carotidien d'une tache vasculaire tumorale. — Les hématomes épiduraux spontanés de la fosse temporale, les artériopathies pré et postcaverneuses, les agénésies complètes ou incomplètes du siphon nous paraissent dorénavant faire partie, du diagnostic différentiel des méningiomes de la petite aile.

Zusammenfassung

Es wird die neuroradiologische Symptomatik der Meningeome des kleinen Keilbeinflügels anhand von Nativ-Aufnahmen und Arteriogrammen beschrieben. Dabei wird hervorgehoben, daß ähnliche röntgenologische Veränderungen auch bei epiduralen Hämatomen der Fossa temporalis, bei Arteriopathien und bei Stenosen im Bereich des Carotissiphons gefunden werden können.
