Summary
Angiographic recognition of cerebral metastases depends on the presence of multiple focal space-occupying processes or areas of pathological circulation which are most clearly visible in the capillary phase. Functional evaluation of the pathological vessels in relation to circulation time enhances the possibilities of differential diagnosis between meningioma, glioma, haemangioma and metastases. — Additional pathological changes in the vessels themselves, e.g. occlusion of the internal carotid artery, may further complicate the diagnosis. In the patient described in this report, correlation of the clinical findings with serial contrast studies resulted in the demonstration of an occipital metastasis by angiographic means, in the presence of occlusion of the ipsilateral internal carotid artery.
Résumé
En angiographie, les métastases cérébrales se caractérisent habituellement par des processus expansifs multiples ou des zones de vascularisation pathologique plus nettement visibles lors de la phase capillaire. L'evaluation fonctionnelle des vaisseaux pathologiques par rapport au temps circulatoire augmente les possibilités de diagnostic différentiel entre méningiomes, gliomes, hémangiomes et métastases. Des modifications associées des vaisseaux eux-mêmes telle que l'occlusion de l'artère carotide interne, peuvent rendre le diagnostic encore plus difficile. Chez le malade présenté dans ce rapport, la corrélation entre l'examen clinique et la sérioangiographie permit de démontrer l'existence d'une métastase occipitale avec occlusion de l'artère carotide interne homolatérale.
Zusammenfassung
Die Erkennung zerebraler Metastasen gelang aus dem Angiogramm beim Vorliegen mehrfacher Lokalisationen raumverdrängender Prozesse oder pathologischer Vascularisationen, wenn diese in der capillären Phase am deutlichsten gezeichnet sind. Eine funktionelle Zirkulationsbeurteilung in den pathologischen Gefäßen ermöglichte eine Verbesserung der Differenzierung zwischen Meningeom, Gliom, Haemangiom und Metastase. — Zusätzliche Veränderungen am Gefäßsystem selbst, wie ein Verschluß der A. car. int. können die Diagnose einer cerebralen Metastase weiter erschweren. Durch das Zusammenwirken klinischer Untersuchungen mit mehrfachen Kontrastmitteldarstellungen gelang der angiographische Nachweis einer occipitalen Metastase beim Verschluß der gleichseitigen A. car. int. bei diesem Patienten.
