Summary
Clinical application has proved radio-isotope myelography with gas (133Xe) a highly valuable diagnostic method. It is indicated when clinical details about the continuity of the subarachnoid space and the level of a block require greater precision. — The basic principles of employing radioactive 133Xe for radioisotope myelography are given. The method is based on determining the location of a subarachnoidally introduced bubble of radioactive gas by means of external gamma radiation registered by counters, scanning device, or gamma-camera. If the cerebrospinal canal is obstructed (tumor, arachnoidal adhesions, median intervertebral hernias, etc.) the radioactive gas is retained at the block. — We have investigated 1500 patients and have found that the method of radioisotope myelography with 133Xe will corroborate and, in a series of cases, improve the topical diagnosis. — It will also establish the presence of a block and its level along the spinal cord but will not, however, reveal the nature of the lesion. — Radioisotope myelography with radiactive gas 133Xe is simple, safe and may be recommended as a routine technique for neurological and neurosurgical clinics.
Résumé
En pratique clinique la myélographie radio-isotopique au gaz 133Xe s'est révélée être une méthode diagnostique de grande valeur. Cet examen est indiqué lorsqu'il s'agit de préciser le tableau clinique et le niveau du blocage. Le principe de la méthode consiste à localiser une bulle de gaz radioactif introduite dans l'espace sousarachnoïdien. Les rayons gamma émis sont enregistrés par des compteurs, des scanners ou par gamma-caméra. En cas d'obstruction du canal cérébro-spinal (tumeur, arachnoïde, hernies médianes etc...) le gaz radioactif stagne au niveau du blocage. Les auteurs ont examiné 1500 malades et prouvé que la myélographie isotopique au 133Xe confirmait, et, dans une série de cas, améliorait le diagnostic de localisation. Cette méthode révèle également la présence d'un blocage et son niveau le long de la moelle épinière mais ne peut cependant pas renseigner quant à la nature de la lésion. La myélographie isotopique au gaz radioactif 133Xe est simple, sûre et peut devenir un examen de routine dans les cliniques neurologiques et neurochirurgicales.
Zusammenfassung
Die Untersuchung mit dem radioaktiven Gas 133Xe wird als einfache und komplikationslose Untersuchungsmethode empfohlen, die als Routine-Technik in neurologischen und neurochirurgischen Kliniken angewandt werden kann. Dabei kann ein Stop nachgewiesen werden, die Art der Läsion läßt sich jedoch nicht bestimmen. Von den Autoren wurden 1500 Patienten mit dieser Methode untersucht.
