Summary
An unusual hereditary tremor which affects only the m. mentalis is reported. In four generations of a West Spanish family 20 members exhibited the symptom. It presents as quick rhythmic contractions of the m. mentalis with a frequency of 6–11/s. The amplitude varies greatly. The trembling of the chin appears at intervals from birth and may last seconds or minutes with shorter or longer asymptomatic periods. Stress or excitement can cause or intensify the symptoms. The quivering of the chin found in this West Spanish family does not differ from that described in 11 other families reported in the literature. They are all clinically identical with each other, and in the 12 families the symptom follows an autosomal dominant mode of inheritance. The penetrance is slightly reduced, and both sexes are equally affected.
It is not serious, but for the patients it is a rather tedious anomaly which is hardly ever reported to a physician.
Zusammenfassung
Es wird über einen seltenen erblichen Tremor berichtet, an dem ausschließlich der M. mentalis beteiligt ist. Das Merkmal wurde in der untersuchten westspanischen Sippe bei zwanzig Angehörigen in vier Generationen beobachtet. Es äußert sich als rasche rhythmische Kontraktion des M. mentalis in einer Frequenz von 6–11 pro Sekunde. Die Amplitude weist größere Schwankungen auf. Das Kinnzittern besteht von Geburt an, zeigt sich immer nur zeitweise, kann Sekunden- bis minutenlang andauern, mit kürzeren oder längeren freien Intervallen. Streß und Erregung können das Kinnzittern auslösen oder verstärken.
Das Kinnzittern in der westspanischen Sippe unterscheidet sich nicht vom Kinnzittern wie es in 11 anderen Sippen in der Literatur beschrieben worden ist. Diese sind auch unter sich klinisch einheitlich.
In allen 12 Sippen wird die Anomalie autosomal dominant vererbt. Die Penetranz ist geringgradig herabgesetzt.
In beiden Geschlechtern kommt erbliches Kinnzittern gleich häufig vor.
Es ist eine harmlose, wenn auch für manche Betroffene lästige Anomalie, die selten zur ärztlichen Kenntnis kommt.
