CT Differential diagnosis and findings in intracerebral haemorrhage

Summary

The CT findings of 73 patients with spontaneous and traumatic intracerebral haematomas are described. The course and pattern of resorption of the bleeding is evaluated. It is shown that the resorption of all haematomas is terminated by the end of week 7 independent of their maximum diameter. Only smaller haemorrhage of less than 2cm can be resorbed in a shorter time. Moreover, the underlying pathophysiologic mechanisms of faster resorption of bleeding located in the basal ganglia and the neighbouring white matter are discussed. The CT findings revealed no clear-cut characteristics for differential diagnosis of the etiology of haematomas, but proved very helpful in defining the prognosis.

Zusammenfassung

Bei 73 Patienten mit spontanen und traumatischen intracerebralen Blutungen werden durch CT-Verlaufsuntersuchungen Gesetzmäßigkeiten der Hämatomresorption aufgezeigt. Nach computertomographischen Beurteilungskriterien ist die Resorption aller intracerebraler Hämatome weitgehend unabhängig von dem Maximaldurchmesser bis zum Ende der 7. Woche ohne Ausnahme abgeschlossen. Lediglich kleinere Blutungen bis zu 2cm Durchmesser wiesen auch schnellere Resorptionsverläufe auf. Die pathophysiologischen Grundlagen des Resorptionsvorganges bei größeren Blutungen sowie die möglichen Ursachen für die schnellere Resorption im Marklager- und Stammgangliengebiet werden besprochen. Ein Vergleich der CT-Merkmale von intracerebralen Blutungen unterschiedlicher Ätiologie zeigt, daß diese keine eindeutige differentialdiagnostische Aussagekraft für die Klärung der Blutungsursache, jedoch prognostische Bedeutung besitzen.
