Summary
81 patients were submitted to repeat myelography using water-soluble contrast media, a total of 188 myelograms. The material is reviewed with respect to the demonstration of leptomeningeal changes.
In 13 patients, 12 of them following the use of methyl-glucamine iothalamate (Conray Meglumine 282) and 1 following sodium methiodal (Abrodil, Conturex), leptomeningeal changes were demonstrated upon repeat myelography in the lumbosacral sac. The radiological findings were uniform and consisted of rounded root pockets and homogeneous contrast with inadequate visualization of fila radicularia. The findings were found to be related to both the dose of contrast medium used and the protein content of the cerebrospinal fluid, while the time interval and operative trauma did not appear to be significant factors. In 4 of the 12 patients with leptomeningeal changes following Conray myelography, a certain relationship was perhaps present between the changes and the clinical symptoms.
The conclusion is to warn against the use of too high a dose of contrast medium and to advise its emptying out in cases with elevated cerebrospinal fluid protein, and in addition perhaps to replace the use of Conray Meglumine by Dimer-X, based on the experimentally demonstrated lower toxicity of the latter.
Résumé
Les auteurs présentent les cas de 81 malades soumis à des myélographies répétées à l'aide de produits de contraste hydrosolubles, soit au total 188 myélographies. Ils étudient en particulier les modifications leptoméningées survenues dans ce cas. Treize malades, dont douze examinés avec du méthyl-glucamine iothalamate (Conray-Méglumine 282) et un malade examiné au sodium méthiodal (Abrodil-Conturex) présentaient des modifications leptoméningées du cul de sac lombo-sacré. Les données radiologiques étaient uniformes et consistaient en tâches radiculaires arrondies et en contraste homogène avec visualisation insuffisante des racines nerveuses. Les symptômes cuniques purent être mis en rapport tant avec la dose de produits de contraste utilisés qu'avec la teneur en protéines du liquide céphalo-rachidien tandis que l'intervalle de temps et le traumatisme opératoire semblaient avoir peu d'importance. — Chez quatre malades des douze malades présentant des modifications leptoméningées après myélographie au Conray, une certaine relation put être établie entre les modifications et les symptômes cuniques. En conclusion, il faut éviter d'utiliser une trop grande quantité de produits de contraste et il est conseillé de le retirer en cas d'une teneur en protéines trop élevé du liquide céphalo-rachidien et de remplacer le Conray-Méglumine par du Dimer X. La faible toxicité de ce produit a été démontré expérimentalement.
Zusammenfassung
Untersuchung der leptomeningealen Veränderungen bei 81 Patienten, bei denen insgesamt 188 Myelogramme durchgeführt wurden. Dïese Veränderungen wurden bei wiederholten Myelographien in der Lumbosacral-Region festgestellt. Da diese Veränderungen nach wiederholten Myelographien immer wieder auftreten, wird darauf hingewiesen, daß die Kontrastmittel-Dosis so niedrig wie möglich gehalten werden soll. Auch soll das Kontrastmittel nach der Myelographie, besonders bei einem erhöhten Eiweißgehalt des Liquors, wieder entfernt werden. Dimer X ist den anderen Myelographie-Mitteln vorzuziehen, da die Gewebsirritation bei diesem Kontrastmittel geringer ist.
