Summary
By careful comparison and correlation of histopathologic and sonographic findings, it could be demonstrated, that transrectal sonography is a useful diagnostic method for evaluating prostatic carcinoma. Comparing the echomorphology of normal prostatic tissue, benign prostatic hyperplasia and carcinoma of the prostate, we found that cancerous tissue shows decreased echo reflexes in 85% of the cases. There is no significant correlation between tumor grade and echo pattern, mainly because of the histologic variability of prostatic cancer. Transrectal sonography is of great value in exploring the extension of the tumor in respect to capsule and seminal vesicle infiltration and penetration of the capsule; it is not suited for detecting incidental carcinoma. Ultrasonically guided punch biopsies were done in 36 patients with T1, T2 and T3 lesions. All patients except one had positive histologic findings.
