Summary
The sensation of a ‘snapping hip’ is due to a tight iliotibial band sliding abnormally over the greater trochanter during certain movements of the hip. Diagnosis is strictly by clinical examination. Radiographs and other investigations are unnecessary, expensive and may be psychologically harmful. We have assessed the results of operation in 35 such hips in 23 patients, reviewed at between 2 and 17 years. A cruciate incision was made in the iliotibial band over the greater trochanter, and the four flaps produced were sutured back on themselves, or to adjacent structures. The results were disappointing, with 30% of patients having a successful result, 30% a recurrence of symptoms and over 60% continuing to experience pain of a varying degree. In the majority of patients operation is not justified, but may be indicated if there is demonstrable abnormality, after trauma or in the presence of severe, intractable pain.

La hanche à ressaut est une anomalie fonctionnelle extra-articulaire liée à la tension excessive de la bandelette de Maissiat qui glisse brusquement sur le grand trochanter lors de certains mouvements. Son diagnostic est uniquement clinique, et les examens complémentaires inutiles et couteux aggravent l'inquiétude ou la sinistrose de ces sujets souvent prédisposés par un terrain psychologique labile. Trente cinq hanches à ressaut ont été opérées chez 23 patients réévalués tardivement, de 2 à 17 ans, en moyenne 7 ans après l'intervention. Les résultats du traitement chirurgical qui a consisté après incision cruciforme de la bandelette de Maissiat, à replier quatre lambeaux aponévrotiques et à les suturer sur eux-mêmes ou aux structures voisines, sont très décevants. 30% seulement des patients sont guéris, 30% récidivent et surtout 60% gardent une gêne ou des douleurs d'intensité variable. L'abstention thérapeutique très précisément et clairement expliquée au patient est donc de mise, au moins initialement. Mais l'existence de lésions anatomo-pathologiques manifestes à l'intervention justifie l'indication chirurgicale dans des cas très sélectionnés, formes traumatiques pures et formes très douloureuses entrainant un handicap objectif notable.
