Summary
The case is described of a 41-year-old man who developed chronic secondary hypoparathyroidism after thyroidectomy. The persistant calcium deficit resulted in a disturbance of CNS function. Predominant symptoms were a psycho-organic syndrome and a personality change leading to temporary alcoholismus and finally resembling a dementia. In addition we found calcification of the basal ganglia and generalised epileptic fits. Treatment with calcium ions alone resulted in an almost complete disappearance of all symptoms with the exception of the intracerebral calcification on cataracta tetanica. The connection between the clinical symptoms and the calcium-dependant functional disturbance of the cell membrane as well as the intracellular state of the neurones and the neuroglia are discussed with reference to most recent knowledge about the biochemical effect of calcium on CNS tissue. The pathogenesis of symmetrical cerebral calcification processes is discussed with reference to new results.
Zusammenfassung
Es wird über einen 41 jährigen Mann berichtet, der nach Strumektomie mit einem chronisch sekundären Hypoparathyreoidismus erkrankte. Der ständige Calciummangel bewirkte erhebliche zentralnervöse Beeinträchtigungen. Im Vordergrund stand ein organisches Psychosyndrom, eine Wesensänderung, die vorübergehend zum Alkoholismus fürhrte und zuletzt wie ein dementiver Abbau wirkte. Hinzu kamen Stammganglienverkalkungen sowie generalisierte epileptische Anfälle. Bis auf die intracerebralen Verkalkungen und die Cataracta tetanica bildeten sich die Symptome auf alleinige Gabe von Calciumionen weitgehend zurück. Die Zusammenhänge zwischen der klinischen Symptomatik und den calciumabhängigen Funktionsbeeinträchtigungen der Zellmembranen und dem Zellinneren von Neuronen bzw. Neuroglia werden an Hand neuerer Kenntnis über die biochemischen Wirkungen von Calcium im zentralnervösen Gewebe diskutiert. Die Pathogenese der symmetrischen cerebralen Kalkablagerungen wird an Hand neuerer Befunde erörtert.
