Summary
1. 2 cases of infantile neuro-axonal dystrophy Seitelberger (InaD) are reported: A 5 1/2-year-old boy (clinical symptoms: idiocy, spastic tetraplegia, hyperkinesia, cerebral convulsions), and a 10-year-old girl (psychomotor retardation, obesity). The outstanding neuropathologic findings are axonal swellings which are most numerous in the dorsal regions of the medulla oblongata (especially of the n. gracilis and cuneatus). There is an extreme generalized cerebellar atrophy in one case.
2. Possible theories regarding pathogenesis and localization of the axonal swellings (proximal or terminal?) are discussed.
3. Common features of and differences between InaD and Hallervorden-Spatz disease (HSD) are demonstrated. Clinico-anatomically InaD (20 cases) and HSD (32 cases of the literature) can be differentiated. Another group of 9 cases shows characteristics of both types. These mixed forms are possible evidence of a common pathogenesis of the underlying disorder in both groups, whose etiology is as yet unknown. — Heredity is important in both groups. In one fourth of the listed cases of InaD siblings were also affected; parents were consanguineous in one fifth of the children.
Zusammenfassung
1. Es werden 2 Fälle von Infantiler neuro-axonaler Dystrophie Seitelberger (InaD) mitgeteilt: ein 5 1/2jähriger Knabe (klinisch: Idiotie, Tetraspastik, Hyperkinesen, epileptische Anfälle) und ein 10jähriges Mädchen (verzögerte psychomotorische Entwicklung, Debilität, Adipositas). — Den auffälligsten neuropathologischen Befund stellen Axonauftreibungen dar; diese sind in dorsalen Gebieten der Medulla oblongata (besonders in den Hinterstrangskernen) am zahlreichsten. In einem Fall besteht eine hochgradige allgemeine Kleinhirnatrophie.
2. Möglichkeiten der Pathogenese der Axonauftreibungen sowie die Frage der Lokalisation der Anschwellungen im Verlaufe des Axon (proximal oder terminal) werden diskutiert.
3. Gemeinsamkeiten und Unterschiede zwischen der InaD und der Hallervorden-Spatzschen Krankheit (HSK) werden im Hinblick auf die histopathologischen Befunde und die klinischen Symptome erörtert. Klinisch-anatomisch lassen sich die Erscheinungsformen der InaD (insgesamt sind uns 20 neuropathologisch untersuchte Fälle bekannt) und der HSK (32 Fälle aus der Literatur) in der Mehrzahl der Beobachtungen voneinander differenzieren. Eine weitere Gruppe von 9 Fällen zeigt Merkmale sowohl der InaD als auch der HSK. Diese „Übergangsformen“ könnten auf Gemeinsamkeiten der Pathogenese des zugrundeliegenden Krankheitsprozesses hinweisen, dessen Ursache unbekannt ist. — Bei beiden Erscheinungsformen spielt Heredität eine Rolle. Bei der InaD waren in einem Viertel der Fälle Geschwister miterkrankt; ein Fünftel der Kinder hatten blutsverwandte Eltern.
