Summary
Hemiballismus was cured with progabide, a new GABA-mimetic agent, in two patients not responding to neuroleptics. The therapeutic effects appeared to be specific and might be related to the putative GABA-ergic neurotransmission involved in subthalamopallidal or striatopallidal pathways.
Zusammenfassung
Zwei Fälle von Hemiballismus, die nicht auf Neuroleptika ansprachen, wurden mit Progabide, einer neuen GABA-mimetischen Substanz, behandelt. Die unwillkürlichen Bewegungen verschwanden bei beiden Patienten unter einer täglichen Dosis von 600 bzw. 900 mg. In einem Fall führten mehrere momentane Unterbrechungen der Behandlung jeweils zu einer prompten Verschlechterung der Symptomatologie. Die Therapie mit Progabide wurde schließlich nach 3 bzw. 47 Tagen abgebaut ohne Wiederauftreten des Hemiballismus. Der mögliche pharmakologische Angriffspunkt des Progabide wird diskutiert im Lichte neuester Angaben über die GABA-ergischen Synapsen im extrapyramidalen System.
