Zusammenfassung
Ein 45jähriger früherer Matrose der deutschen Handelsmarine gelangte wegen einer chronisch verlaufenden cerebralen Erkrankung mit Störungen im Sinne eines Angularissyndroms zur stationären Aufnahme. Ein raumfordernder Prozeß oder Verschluß größerer Gefäße konnte durch die neuro-radiologische Diagnostik ausgeschlossen werden. Als Ursache der bereits ein Jahr vor Beginn der Hirnstörungen auftretenden flüchtigen Schwellungen an verschiedenen Körperstellen und einer starken Eosinophilie im Blut konnte eine Loa Loa Filariosis durch den stark positiven Ausfall serologischer Reaktionen, den Nachweis von Mikrofilarien im Blut und die Excision einer Makrofilarie aus einer Calabarschwellung am rechten Ellenbogen gesichert werden. Unter Hinweis auf ähnlich gelagerte Fälle in der Literatur wird das cerebrale Krankheitsbild als Komplikation der Loa Loa-Infektion erklärt, wobei die Frage einer mechanischen Verlegung von Hirngefäßen oder einer allergischen Encephalitis pathogenetisch diskutiert wird. Eine Encephalitis wird für wahrscheinlicher gehalten. Bei der geschilderten Erkrankung handelte es sich um den ersten Fall einer Berufskrankheit durch cerebrale Komplikationen einer Filariosis Loa Loa, der in Deutschland zur Entschädigung gelangte.
