Summary
Nystagmographic controls of 29 cases with unilateral (idiopathic) Bell's palsy have given the following results:

22 (75%) patients have shown objective symptoms of a vestibular disorder whereas only 3 (10%) patients have complained of vertigo. The origin of the vestibular dysfunction is probably a transient lesion of the peripheral neuron (viral or vascular).

In early cases (interval between onset of palsy and vestibular examination 6–24 days) vestibular symptoms are the following: spontaneous nystagmus to the opposite side of the lesion (50%), reduced caloric responses on the side of the lesion (70%), directional preponderance of galvanic responses towards the unimpaired side (70%).

In late cases (interval 25–240 days) the following symptoms have been found: spontaneous nystagmus towards the side of the paresis (35%), caloric esponses mostly normal, directional preponderance of galvanic reactions towards the side of the lesion (> 90%).

The changes of the vestibular symptomatology are explained by the phenomena of central compensation and recovery of peripheral function due to the restitution of nerve conductivity.

The galvanic test seems to be more sensitive than any other vestibular test with respect to an early and precise recognition of changes within the vestibular system indicating the site of the lesion, the onset of central compensation and functional restitution of the peripheral neuron.
Zusammenfassung
Die nystagmographische Kontrolle von 29 Fällen mit Vorliegen einer einseitigen, peripheren, idiopathischen Facialislähmung hat folgendes ergeben:

22 Patienten zeigten die objektiven Symptome einer vestibulären Funktionsstörung, wogegen subjektiv nur in 3 Fällen Schwindelerscheinungen bestanden. Als wahrscheinliche Ursache dieser vestibulären Funktionseinbuße wurde eine vor bestehende Läsion des peripheren Vestibularisneurons in Erwägung gezogen (viraler oder vaskulärer Genese).

In Frühfällen (Zeitabschnitt zwischen Lähmungsbeginn und otoneurologischer Abklärung 6–24 Tage) fanden sich folgende Symptome: Spontannystagmus zur gesunden Seite (50%), calorische Untererregbarkeit auf der Seite der Lähmung (70%), erhöhte Reizschwelle des galvanisch ausgelösten Nystagmus auf der Seite der Facialisparese und Richtungsüberwiegen zur gesunden Seite in 40 bzw. 70%.

In Spätfällen (Zeitintervall 25–240 Tage) konnte ein Spontannystagmus zur Seite der Lähmung in 35% der Fälle nachgewiesen werden, ein Richtungsüberwiegen des galvanischen Nystagmus (ohne Schwellenveränderung) zur Seite der Läsion in > 90%, während die calorischen Reaktionen mit wenigen Ausnahmen normal und symmetrisch ausfielen.

Die Änderung der vestibulären Symptomatologie, welche beim Vergleich der Frühfälle mit den Spätfällen beobachtet werden konnte, wurde durch zentrale Kompensationsvorgänge und die Wiederherstellung der peripheren Nervenleitfähigkeit, d. h. die Rükehr der normalen vestibulären Funktion, erklärt.
