Die wichtigkeit der pneumatisation des attiks bei auto- und allogenen tympanoplastiken

Summary

The attic is origin and location of most of the chronic inflammatory processes of the middle-ear. Therefore, they can be operated most conservatively when starting from this point. When proceeding in this way, the lateral attic-wall and, if necessary, part of the post-meatal wall has to be exactly reconstructed in order to ensure repneumatisation of all the middle-ear spaces. For this purpose, it is advantageous to make an impression in wax and to form the bony implant accordingly. Also, forming a new attic is inevitable in order to guarantes normal function of a combined homologue implant of drum and ossicles.

Zusammenfassung

Der Attik ist Ausgangspunkt und Sitz der meisten chronischentzündlichen Prozesse des Mittelohres, die daher am schonendsten von bier aus angegangen werden. Um nach einem sanierenden Eingriff die pneumatischen Räume des gesamten Mittelohres wieder zu sichern, muß die laterale Attikwand und gegebenenfalls auch ein angrenzender Teil der hinteren Gehörgangswand exakt wiederaufgebaut werden. Für diesen Zweck empfiehlt rich die Anfertigung eines Abdruckes, nach dem das Knochen-Implantat geformt werden kann. In gleicher Weise muß der Attik wiederaufgebaut werden, um die Funktion eines kombinierten Homoio-Transplantates aus Trommelfell und Gehörknöchelchen zu garantieren.
