Summary
Tympanoplasty in an obstructed tympanic space can be successful, if a paraffin prosthesis (melting point 47°C) is used after tympanolysis to restore a permanent air-containing middle ear cavity. After 6–8 weeks the paraffin is removed. New mucosa grows from the mouth of the Eustachian tube around the paraffin prosthesis. Very often the nasopharyngeal tube must be treated by small plastic-tubes (Tubentamponade).
In 62 cases paraffin prostheses have been used. 40 of these cases reached a remarkable hearing gain and an air-containing middle ear. 6 ears are still under treatment. In 16 cases hearing could not be improved. There was no loss of bone conduction. Some cases needed repeated replacement of paraffin.
Zusammenfassung
Temporäre Paraffineinlagen (Schmelzpunkt 47°C) in die von Narben befreite und — wenn nötig — mit einem Transplantat ergänzend abgedeckte Pauke ermöglichen innerhalb von 6–8 Wochen das Einwachsen neuer Schleimhaut. Die neue Mucosa stammt von Schleimhautresten oder aus der Tube. Fehlende oder mangelhafte Tubenfunktion wird durch Tubentamponaden (manchmal in Kombination mit intratubaren Bestrahlungen) wiederhergestellt.
Von 62 wegen Narbenprozessen in der Pauke operierten Ohren erreichten 40 einen guten Horgewinn. 6 Patienten sind noch in Behandlung, bei 16 Fallen konnte das Gehör nicht nennenswert gebessert werden. Ein operativ bedingter Innenohrschaden ist in keinem Fall aufgetreten. In Einzelfällen wurde der Horgewinn erst nach wiederholter Paraffineinlage erreicht.
