Summary
Cystinosis, an autosomal recessively inherited disease, with cystine crystals in the conjunctiva, bone marrow, lymph nodes and reticuloendothelial cells, occurs in at least three different forms. The nephropathic type is clinically characterized by progressive renal tubular failure with Fanconi syndrome and serious rickets. The excessive glomerular damage leads to uremic death within the first decade of life. In adults, cystinosis is seen as a completely benign variant. Patients with the late-onset (intermediate) type show an attenuated form of the disease that allows them to survive into the second or third decade of life. The characteristic patchy depigmentation in the peripheral retina is found consistently in the nephropathic form, in some cases in the late-onset form, but never in the benign form. Rickets is present in the nephropathic and in the late-onset variant, but is absent again in the benign type. Cystine crystals in the bone marrow and conjunctiva are features common to all three forms of the disease. Though the primary biochemical defect in cystinosis is still unclear, it has been demonstrated that leukocytes and cultured fibroblasts of patients with all three forms of cystinosis have elevated free cystine levels. The concentrations range from 30 times normal in the benign up to 100 times normal in the nephropathic disorder. The late-onset type falls between these ranges. Heterozygotes for all three types show modest increases in cystine concentration. An autosomal recessive pattern of inheritance seems to be implicated in the three forms of cystinosis. Clinical examination of pedigrees are substantiated by biochemical proof of elevated free cystine in leukocytes and fibroblasts from heterozygotes for the different forms. Genetic heterogeneity is suggested by the differences in clinical and biochemical expression in different families. Experimental treatment with a diet restricted in cystine and methionine and administration of D-penicillamine failed to influence the course of the disease. Dithiothreitol has been suggested as a useful form of therapy, but is still in an experimental stage. At present, renal homotransplantation performed in children with nephropathic and intermediate type of cystinosis appears to offer the best opportunity for extended survival of these children and is the only effective treatment in terminal renal failure.
Prenatal diagnosis of the homozygous status of nephropathic cystinosis has been established by measurement of the elevated cystine in cultured amniotic fluid cells by a pulse-labeling technique, making preventive control of the disease possible.

Die Cystinose, ein autosomal-recessives Erbleiden, ist charakterisiert durch Ablagerung von Cystinkristallen in der Conjunctiva, im Knochenmark und in Zellen des reticuloendothelialen Systems. Drei Phänotypen wurden bisher beobachtet. Der nephropathische Typ ist klinisch durch progrediente Nierentubulusstörung mit den Zeichen des Fanconi-Syndroms und schwerer Vitamin D-refraktärer Rachitis gekennzeichnet. Die fortschreitende glomeruläre Schädigung der Niere führt in der Regel innerhalb des 1. Lebensjahrzehntes zum Tod an Urämie. Die Cystinose der Erwachsenen ist eine gutartige Variante. Die intermediäre Form der Cystinose weist ein etwas milder ausgeprägtes klinisches Bild als die nephropathische Form auf. Sie führt aber ebenfalls zur terminalen Niereninsuffizienz. Die für die nephropathische Cystinose typische Retinopathie wird auch bei einigen Patienten des intermediären Typs beobachtet. Sie fehlt jedoch stets bei der benignen Variante. Cystinkristalle im Knochenmark und der Conjunctiva werden bei allen drei Formen der Krankheit beobachtet. Der primäre Defekt der Cystinose ist noch ungeklärt. Mit biochemischen Methoden läßt sich jedoch ein erhöhter freier (nicht proteingebundener) Cystinspiegel in Leukocyten und Fibroblasten nachweisen. Die Erhöhung reicht vom 30fachen bei der benignen bis zum 100fachen bei der nephropathischen Form. Die Werte für den intermediären Typ liegen dazwischen. Die Heterozygoten für alle drei Phänotypen haben mäßig erhöhte Cystinwerte (4-7fach). Der Erbgang ist bei allen drei Formen autosomal-recessiv. Durch Stammbaum-untersuchungen der einzelnen Familien und den biochemischen Nachweis der Heterozygoten wurde der Erbgang aufgeklärt. Die unterschiedliche klinische und biochemische Ausprägung der Cystinose läßt Heterogenie vermuten.
Die Therapie der Kinder mit einer cystin- und methioninarmen Diät zeigte ebenso wie der Versuch einer Behandlung mit D-Penicillamin keinen Einfluß auf den fatalen Verlauf der nephropathischen Krankheitsform. Im terminalen Stadium der Niereninsuffizienz wurden Kindern sowohl mit der nephropathischen als auch mit der intermediären Form der Cystinose Nieren transplantiert. Diese Behandlung scheint im Augenblick eine gute Chance für ein längeres Überleben der Kinder zu bieten.
Die pränatale Diagnostik der nephropathischen Cystinose wurde in kultivierten Amnionzellen durchgeführt. Der homozygote Status wurde durch radioaktive Pulsmarkierung bestimmt und nach erfolgter Schwangerschaftsunterbrechung fetales Gewebe eingehend biochemisch und histologisch untersucht.
Hybridisierungsversuche cystinotischer mit nichtcystinotischen menschlichen Zellen scheinen eine Möglichkeit zu bieten, der Aufklärung des primären Defektes der Cystinose näherzukommen.
