Summary
The mode of action of bleomycin after intraarterial perfusion in 53 patients with advanced squamous cell carcinoma of the oral cavity is reported on the basis of clinical results and pathologico-anatomic findings. In 29 out of 32 cases not previously treated a clinically complete remission could be achieved by primary chemotherapy. In 8 cases these findings were histomorphologically confirmed and 7 patients have shown a persistant remission for 9 to 61 months. The chances of success are much less favorable in the treatment of recurrences. No clinical advantages were observed when combining bleomycin with other chemotherapeutics.
The effect of bleomycin on keratinizing squamous cell carcinomas could be defined in more histo-morphological detail: The tumor cells die under forced keratinization. A resorptive, granulomatous inflammation with foreign body giant cells and finally a connective tissue scar are the next steps. This particular mechanism explains the selective reaction of highly differentiated keratinizing squamous cell carcinomas to bleomycin and the negative results obtained with non-differentiated tumors and recurrences. The monocytogenous origin of the giant cells was electron microscopically demonstrated and thus their former interpretation as tumor giant cells refuted.
Side-effects of bleomycin are observed especially in the integument, the region of the perfused vessels, and in the lungs.
The endothelium of the vessels is damaged, swells and vacuolarly degenerates, parts of cytoplasma are lost into the vascular lumen and the elastic lamellae are partly destroyed. This damage is the basis for the development of thrombi which may lead to necrosis in the region of perfusion.
In the lungs an alveolitis develops with intraalveolar and interstitial edema, pulmonary hyaline membranes, microthrombi, intraalveolar and interstitial fibrosis as well as atelactasis and meta- and dysplasias of the alveolar epithelial cells. The combined effect of many factors produces the clinical picture of the “bleomycin lung”. The extent of the lung damage largely exceeds that of a lung fibrosis as which it was formerly classified. On the basis of these findings, a theory of the parthogenesis of the so-called bleomycin lung is presented. Pertinent prophylaxis is attempted by anticoagulation therapy and differentiated functional examinations of the lungs to early diagnose damage caused by bleomycin.
Zusammenfassung
Über die Wirkungsweise von Bleomycin nach intraarterieller Perfusion bei 53 Patienten mit fortgeschrittenen Plattenepithelcarcinomen der Mundhöhle wird anhand von klinischen Ergebnissen und pathologisch-anatomischen Befunden berichtet. Bei 32 nicht vorbehandelten, primär cytostatisch therapierten Fällen konnte 29 mal eine klinisch komplette Remission erreicht werden. 8 mal wurden diese Befunde histo-morphologisch bestätigt, darüber hinaus befinden sich 7 Patienten in einer anhaltenden Remission mit einer Dauer von 9 bis 61 Monaten. Die Erfolgsaussichten bei der Behandlung von Rezidivtumoren sind sehr viel ungünstiger. Eine Kombination von Bleomycin mit anderen Chemotherapeutika wies klinisch keine Vorteile auf.
Histo-morphologisch konnte die Wirkungsweise des Bleomycins auf Plattenepithelcarcinome mit Verhornungstendenz näher definiert werden. Das Tumorareal wird unter forcierter Verhornung devitalisiert. Es folgt eine resorptive, granulomatöse Entzündung mit Fremdkörperriesenzellen und schließlich eine bindegewebige Organisation. Dieser besondere Reaktionsmechanismus erklärt das selektive Ansprechen der hoch differenzierten, verhornenden Plattenepithelcarcinome auf Bleomycin und die schlechten Resultate bei entdifferenzierten Tumoren und Rezidiven. Elektronenoptisch wurde die monozytogene Herkunft der Riesenzellen bewiesen und somit die bisherige Deutung als Tumorriesenzellen widerlegt.
Die Nebenwirkungen des Bleomycins manifestieren sich in besonderem Maße am Integument, am perfundierten Gefäßareal und an der Lunge.
An den Gefäßen kommt es zu Endothelschäden mit Schwellung und vacuoliger Degeneration sowie Abspaltung von Cytoplasmaanteilen in das Gefäßlumen hinein und teilweise zur Zerstörung der elastischen Lamellen. Diese Schädigung bildet die Grundlage für die Ausbildung von Thromben, die im Perfusionsareal zu Nekrosen führen können.
An der Lunge entsteht eine Alveolitis mit intraalveolärem und interstitiellem Ödem, pulmonalen hyalinen Membranen, Mikrothromben, intraalveolärer und interstitieller Fibrose sowie Atelektasen und Alveolardeckzellmeta- und -dysplasien. Durch das Zusammentreffen einer Vielzahl von Faktoren kommt es zu dem Bild der Bleomycin-Lunge. Der Umfang dieses Lungenschadens geht über das bisher als Lungenfibrose bezeichnete Maß weit hinaus. Anhand dieser Befunde wird eine Vorstellung zur Pathogenese der Bleomycin-Lunge zur Diskussion gestellt. Ansätze für eine Prophylaxe ergeben sich durch eine Antikoagulantientherapie und durch differenzierte Lungenfunktionsproben zur Früherkennung des Bleomycinschadens.
