Combination chemotherapy in patients with disseminated bronchogenic carcinoma

Summary

49 patients with disseminated bronchogenic carcinoma (small cell, 16; squamous cell, 17; large cell, 12; adeno, 4) were treated with a combination of adriamycin, cyclophosphamide, vincristine and DTIC. In a randomized series (21 patients) chemotherapy alone was compared to chemotherapy plus heparin. Partial remission was achieved in 8 patients with small cell carcinoma, in 5 patients with large cell carcinoma and in 2 patients with squamous cell carcinoma. The survival of patients who responded to therapy was greater than the survival of patients who did not respond to therapy. The duration of remission and the survival time was not longer in patients who had additional heparin therapy.

Zusammenfassung

49 Patienten mit metastasiertem Bronchialcarcinom (16 kleinzellige Carcinome, 17 Plattenepithelcarcinome, 12 großzellige Carcinome, 4 Adenocarcinome) wurden mit einer Kombinationschemotherapie bestehend aus Adraimycin, Cyclophosphamid, Vincristin und DTIC behandelt. Zusätzlich erhielten in einer Pilot-Studie 21 Patienten nach Randomisierung eine Anticoagulantientherapie mit Heparin oder wurden nicht anticoaguliert. Bei 8 Patienten mit kleinzelligem Carcinom, 5 Patienten mit großzelligem Carcinom und 2 Patienten mit Plattenepithelcarcinom konnte eine Teilremission erzielt werden. Eine weitere Anzahl von Patienten zeigte ein stationäres Tumorwachstum über mehrere Monate. Die mittleren Überlebenszeiten waren bei Patienten, die auf die Therapie ansprachen, deutlich länger als bei Therapieversagern. Ein zusätzlicher günstiger Effekt der Anticoagulantientherapie wurde nicht gesehen.
