Summary
In 56 patients with Hodgkin's disease, the following bloodtests were carried out: erythrocyte sedimentation rate (ESR), fibrinogen, α2-globuline, serium iron concentrations and alkaline phosphatase activity. In some patients we additionally measured alkaline leucocyte phosphatase and serum ribonuclease activity.
In our series ESR, serum iron and α2-globuline concentrations were the most sensitive metabolic parameters. A rise in fibrinogen concentration, alkaline phosphatase and serum ribonuclease activity seems to indicate extensive disease.
It is not possible, however, to discern between a state of remission and stage I by means of these parameters. ESR, serum iron and α2-globuline concentrations might be either elevated or normal in both instances. These parameters seem important in order to distinguish between a remission or stage I on the one hand and extensive disease in stage III and IV on the other hand. Concomitant findings of ESR above 40 mm/h, elevated concentrations of fibrinogen and α2-globuline, as well as elevated alkaline phosphatase and serum ribonuclease activity mostly indicate stage III or IV.
Zusammenfassung
Bei 56 Patienten mit Morbus Hodgkin wurden Blutkörperchensenkung (BKS), Fibrinogenkonzentration, α2-Globulinkonzentration, Eisenspiegel und Aktivität der alkalischen Phosphatase im Plasma bestimmt. Bei einem Teil dieser Patienten wurde außerdem die Aktivität der alkalischen Leukocytenphosphatase und der Ribonuclease im Serum gemessen.
Die empfindlichsten metabolischen Parameter sind BKS, Eisenspiegel und α2-Globulinkonzentration. Erst bei ausgedehnterem Hodgkinbefall steigen dann auch Fibrinogenkonzentration, Aktivität der alkalischen Phosphatase im Plasma und der Ribonuclease im Serum an.
Eine Differenzierung zwischen Remission und Stadium I ist durch Bestimmung dieser metabolischen Parameter nicht möglich. BKS, Eisenspiegel und α2-Globulinkonzentration können in beiden Stadien normal oder pathologisch sein. Wichtig erscheint die Bestimmung dieses Spektrums metabolischer Parameter zur Unterscheidung zwischen Remission oder lokalem HodgkinBefall im Stadium I einerseits und ausgedehntem Befall im Stadium III und IV andererseits. Eine Erhöhung der BKS über 40 mm nach einer Stunde zusammen mit erhöhter Fibrinogenund α2-Globulinkonzentration und erhöhter Aktivität der alkalischen Phosphatase und der Ribonuclease finden sich meist im Stadium III oder IV.
