The recurrence of pain following operation for herniated lumbar disc

Fresh herniation or extradural scar tissue?

Summary

The clinical findings, the appearance of the myelogram and the findings at re-operation are described in 34 patients who had persistent or recurrent pain following operation for removal of a herniated lumbar disc. An attempt was made to distinguish before re-operation whether the residue symptoms were due to recurrence of, or a fresh disc herniation, or to extradural scarring. Persistence of pain after the initial operation or the time interval before pain re-appeared did not help to distinguish the causative pathology. A recurrent disc herniation was found if a different disc was involved, but was else best distinguished by myelographic demonstration of a short indentation in the contrast column at the level of a disc or a short root sleeve with a thickened associated nerve root. An absence of indentation in the column suggested extradural scarring. At re-operation disc herniation and extradural scarring without herniation were found in approximately equal numbers of patients.

Résumé

Les auteurs décrivent les signes cliniques, les images myélographiques et les constatations faites à l'occasion d'une réintervention chez 34 malades qui présentaient une douleur persistante ou itérative après une opération pour ablation de hernie discale lombaire. Ils ont tenté de distinguer, avant la réintervention, si les symptômes résiduels étaient le fait d'une récidive ou d'une nouvelle hernie discale ou des tissus cicatriciels extra-duraux. La persistance de la douleur après l'opération initiale ou la durée d'un intervalle de temps entre celle-ci et la réapparition des douleurs ne fournissent pas d'argument au diagnostic étiologique. Une récidive de la hernie discale est mieux objectivée par la myélographie lorsqu'elle montre une petite encoche de la colonne de contraste au niveau d'un disque ou un raccourcissement des culs de sac avec épaississement des racines. Une absence d'encoche de la colonne opaque est en faveur d'une cicatrice extra-durale. Lors de la réintervention, hernie discale et cicatrice extra-durale ont été trouvées dans un nombre de cas approximativement égal.
