Summary
The authors have reviewed the clinical and radiological results of 70 intertrochanteric osteotomies in 65 patients. The mean follow-up was 8 years and most of the patients (53 osteotomies) were examined from 6 to 15 years after operation. The mean age at operation was 47 years. In almost two-thirds of patients the osteoarthritis was secondary to congenital disorders of the hip. The technique was usually a rotational osteotomy with slight medial displacement. The clinical results were satisfactory in 49 osteotomies and unsatisfactory in 21. The radiological results were less encouraging, but an impression was gained that a poor radiological appearance does not necessarily preclude a good clinical outcome. There was no significant statistical difference in the results between primary and secondary osteoarthritis. No clear patterns could be established to explain why the operation succeeded in some patients and failed in others.
Résumé
Les auteurs ont passé en revue les résultats cliniques et radiologiques de 70 ostéotomies intertrochantériennes (65 malades), suivies 8 ans en moyenne. La majorité des malades (53 ostéotomies) ont subi l'examen de contrôle de 6 à 15 ans après l'opération. L'âge moyen des malades était de 47 ans, ce qui est une période critique pour la réalisation d'une arthroplastie totale. Dans presque deux tiers des cas il s'agissait d'une coxarthrose secondaire à une dysplasie congénitale. La technique a généralement consisté en une ostéotomie de rotation avec une légère translation interne. Les résultats cliniques ont été satisfaisants dans 49 ostéotomies et non satisfaisants dans 21. Les résultats radiologiques paraissaient moins encourageants, mais il semble bien qu'un aspect radiologique médiocre n'est pas nécessairement le prélude d'une évolution clinique défavorable. Il n'y a pas de différence statistiquement significative entre les coxarthroses primitives et secondaires. Il n'a pas été possible d'établir clairement les raisons pour lesquelles l'intervention aboutit à un succès ou à un échec.
