Summary
Plasma-cell osteomyelitis is a histological diagnosis and, in the opinion of some authors, is a disease entity which can be differentiated from the sclerosing osteomyelitis of Garré and Brodie's abscess. Our investigation of 12 patients with plasma-cell osteomyelitis leads us to a different conclusion. The radiological appearances are not specific and do not allow a definitive diagnosis of the type of osteomyelitis or the exclusion of other bone disorders. Although there were always an increased number of plasma cells in the specimens from our patients, there were frequently transitional appearances varying from sclerosing osteomyelitis to Brodie's abscess. For this reason, we suggest that the term “primary chronic osteomyelitis” should be used. This makes it possible to distinguish the condition from acute and secondary osteomyelitis as well as from other diseases, such as bone tumours. All our patients were treated by local resection of the osteomyelitis lesion and they all recovered uneventfully; there were no local recurrences.
Résumé
L'ostéomyélite plasmocellulaire représente un diagnostic histologique qui, d'après l'opinion de certains auteurs, devrait être considérée comme une entité nosologique distincte de l'ostéomyélite sclérosante de Garré et de l'abcès de Brodie. D'après nos investigations concernant 12 patients avec le diagnostic histologique «d'ostéomyélite plasmocellulaire» qui ont été traités dans notre hôpital au cours des 10 dernières années, nous pensons qu'une telle entité nosologique n'est pas justifiée. Les aspects radiologiques de nos patients avec une prétendue «ostéomyélite plasmocellulaire» diffèrent beaucoup entre eux et ne permettent donc pas une différenciation exacte ni par rapport à l'ostéomyélite chronique ni par rapport à d'autres maladies osseuses. En outre, parmi nos patients chez qui un nombre accru de plasmocytes a été trouvé à l'examen histologique, nous avons fréquemment observé des cas de transition avec l'ostéomyélite sclérosante ou avec l'abcès de Brodie. Pour cette raison nous proposons le termed «d'ostéomyélite chronique primaire». De cette façon il est possible de distinguer cette maladie de l'ostéomyélite aigüe et de l'ostéomyélite secondaire d'une part, et d'autre part de maladies différentes, avant tout des tumeurs osseuses. L'évolution clinique de nos patients n'a jamais présenté de problèmes particuliers, tous ont été traités par résection locale du foyer. Nous n'avons pas noté de récidives locales.
