Stable pubic fractures in the elderly

The association with sacro-iliac injuries

Summary

A consecutive series of 17 elderly patients with radiological or clinical suspicion of pubic fractures were investigated with bone scintigraphy using 99mTc-methylene diphosphonate. All had an increased isotope uptake over the pubic bone and also over the sacro-iliac region, except in one patient where the sacro-iliac uptake could not be assessed. The isotope uptake was slightly increased in nine patients and markedly so in seven. Computed tomography was performed in three cases. In two who showed a marked increased isotope uptake, fractures adjacent to the sacro-iliac joint could be demonstrated, and in one with only slightly increased uptake, no fracture was found.

Résumé

Dix-sept malades âgés chez qui on pouvait suspecter, sur des signes cliniques ou radiologiques, une fracture du pubis, ont bénéficié d'une scintigraphie au méthylène-diphosphonate de Tc99m. Tous présentaient une hyperfixation au niveau du pubis et également de la région sacro-iliaque, sauf dans un cas où la fixation sacro-iliaque n'a pu être estimée. La fixation de l'isotope était légérement augmentée chez neuf malades et fortement chez les sept autres. On a réalisé un examen tomodensitométrique dans trois cas. Deux fois, alors qu'il existait une notable hyperfixation, on a pu mettre en évidence une fracture au voisinage de l'articulation sacro-iliaque. Dans le 3ème cas, où l'hyperfixation était seulement modérée, il n'y avait pas de fracture.
