The technique is described and a series of 30 cases is reviewed, 8 months to 3 years after operation.
Spondylolysis is seen as an unstable pseudoarthrosis and back pain in spondylolisthesis is considered to arise from vertebral instability so that sound fusion is required in both circumstances. A two-stage technique is proposed for severe spondylolisthesis, using anterior and posterior approaches separated by an interval of 10 days.
Through the anterior approach, removal of the disc is followed by reduction of the displacement, using special instruments, and a fibular bone graft is inserted. Technical details are described to avoid damage to prevertebral vessels and nerves. Second stage posterior fusion consists of bone grafts to the isthmus and posterior intervertebral joints and stabilisation with a special plate placed between the pedicles of the fifth lumbar vertebra and the alae of the sacrum.
The results are excellent, with sound fusion and relief of pain.
A technique for reconstruction of the isthmus without spinal fusion is proposed.
Résumé
Les auteurs présentent, à propos d'une série de 30 cas, avec un recul de 3 ans à 8 mois, leurs techniques de réduction avec ostéosynthèse-arthrodèse stable des spondylolyses et des spondylolisthésis. Considérant que la spondylolyse crée une pseudarthrose instable, ils pensent que les douleurs des spondylolisthésis par lyse isthmique sont dues à l'instablilité vertébrale et ils en déduisent la nécessité d'une excellente fusion.
Ils proposent une technique de double spondylodèse faite en deux temps, antérieur et postérieur, à 10 jours d'intervalle. Le temps antérieur prévoit, dans les cas de spondylolisthésis très déplacés, une réduction per-opératoire après discotomie grâce à leur instrumentation personnelle. Des précisions techniques sont données sur les vaisseaux et les nerfs pré-vertébraux pour éviter tout incident per et postopératoire. L'arthrodèse antérieure utilise une greffe de péroné. L'arthrodèse postérieure avec greffe prélevée in situ et arthrodèse de la région isthmique et des articulations postérieures est stabilisée par une plaque monobloc personnelle, fixée dans les pédicules de L 5 et les ailerons sacrés.
Les résultats présentés sont excellents, aussi bien sur la disparition des douleurs que sur la constance de la consolidation. Une technique de reconstitution isthmique est proposée, sans arthrodèse.
