Bone and joint infections

Summary

Bone and joint infections have a world wide distribution and cause enormous problems in all continents. Since these problems may differ in individual countries, only the universal principles of pathogenesis, diagnosis and treatment of these infections are discussed. It is emphasized that an acute osteomyelitis or arthritis must be treated within one to three days if complications are to be avoided. Treatment of chronic infection must only commence after thorough analysis and planning. The principles for choice and use of antibiotics and for operative treatment are discussed. Some special details of diagnosis and treatment of septic arthritis are given.

Résumé

Les infections ostéo-articulaires s'observent dans le monde entier et causent partout de graves problèmes. Etant donné que ces problèmes peuvent varier d'un pays à l'autre, les auteurs n'envisagent que les principes généraux concernant la pathogénie, le diagnostic et le traitement de ces infections.

Ils insistent sur le fait que les ostéomyélites et les arthrites aigües doivent être traitées dans les tout premiers jours, pour éviter les complications. Le traitement des infections chroniques ne doit être entrepris qu'après un bilan complet et l'établissement d'un plan thérapeutique précis. Ils discutent également des principes qui régissent le choix et l'utilisation des antibiotiques et du traitement chirurgical. Ils terminent en donnant quelques détails particuliers quant au diagnostic et au traitement des arthrites septiques.
