Transcutaneous meniscectomy under arthroscopic control

Summary

A technique of arthroscopic excision of a torn meniscus has evolved during two years' experience of therapeutic arthroscopy of the knee joint. The same method is used for both medial and lateral meniscus lesions under full visual control throughout the procedure, with a standard arthroscope inserted centrally through the patellar tendon. The first 18 consecutive patients thus treated have been followed up. Endoscopic operation was complemented by arthrotomy in three; in 15 no other treatment was given. The time in hospital, convalescence, sick leave, and knee function at follow-up were compared in these 15 patients with matched controls treated in the ordinary way by arthrotomy. Knee function did not differ between the groups, whereas all other variables showed better results after arthroscopic excision.

Résumé

La technique d'excision arthroscopique d'un ménisque déchiré a évolué en deux ans d'arthroscopie thérapeutique du genou. La même méthode est utilisée tant pour les lésions du ménisque interne que pour celles du ménisque externe, à l'aide d'un arthroscope standard introduit au travers du tendon rotulien, permettant un contrôle visuel complet tout au long de l'intervention. Nous avons suivi les 18 premiers malades ainsi traités. L'endoscopie a dû être complétée par une arthrotomie chez trois de ces malades; aucun autre traitement n'a été nécessaire chez les 15 autres. Chez ces 15 malades, nous avons comparé les durées d'hospitalisation, de convalescence et d'arrêt de travail, ainsi que la valeur fonctionnelle du genou avec les malades traités de façon conventionnelle par arthrotomie. Tandis que la fonction du genou ne diffère pas entre les deux groupes, tous les autres paramètres sont meilleurs aprés traitement arthroscopique.
