Complications in surface replacement arthroplasty of the hip: Experience with the paltrinieri — trentani prosthesis

Summary

A method of surface replacement arthroplasty of the hip joint is described for use in selected patients, often in the younger age group. One hundred and forty patients were reviewed after a mean interval of four years. Seventy-two per cent were graded 5 or 6 on the Charnley scale, the remainder had symptoms due to femoral neck fracture or loosening of the prosthetic components. In the main the bad results were attributed to incorrect patient selection or technical imperfections with the operative technique.

The advantages of the operation are no alteration in the relationship of the femoral head, neck and acetabulum and minimal interference with the surrounding muscle and soft tissues. The amount of implanted material is reduced and no entry is made into the femoral medullary canal, which facilitates any further revision surgery that may be necessary.

Description d'une méthode d'arthroplastie «de surface» de la hanche, qui doit être réservée à des malades choisis, notamment parmi les jeunes. Cent quarante opérés ont été revus avec un recul moyen de 4 ans. Soixante douze pour cent ont pu être évalués à 5 ou 6 selon la cotation de Charnley, les autres présentent des symptômes en rapport soit avec une fracture du col fémoral, soit avec un descellement des pièces prothétiques. Dans l'ensemble, les mauvais résultats sont le fait d'indications mal posées ou d'imperfections de la technique opératoire.

Les avantages de cette intervention sont la conservation des rapports entre tête fémorale, col et cotyle et le faible retentissement sur les muscles et les parties molles. Le volume de l'implant est peu important et il n'y a pas de pénétration dans le canal médullaire du fémur, ce qui facilite une éventuelle reprise chirurgicale, si celle-ci s'avère nécessaire.
