Zusammenfassung
Der Einfluß von Cocain, Procain und Lidocain auf das Ruhe- und Aktionspotential, auf die intracelluläre K+-, Na+- und Cl−-Ionenkonzentration sowie auf den Wassergehalt isolierter Froschsartoriusmuskeln wurde untersucht. Die drei Lokalanaesthetica bewirkten im Konzentrationsbereich von 10−4 bis 10−3 g/ml eine dosisabhängige, langsam fortschreitende Senkung des Ruhepotentials, die voll reversibel war. Das Aktionspotential der Muskeln wurde durch Lokalanaestheticadosen von 10−4 bis 4·10−4 g/ml in charakteristischer Weise verändert; unter anderem wurde eine Verkleinerung der Aktionspotentialamplitude sowie Verminderung der maximalen Anstiegs- und Abfallsraten beobachtet; auch diese Veränderungen waren voll rückbildungsfähig. Die im Ringer-Milieu auftretenden K+-Verluste und Na+-Gewinne der Zelle werden durch die Lokalanaesthetica eingeschränkt. Die Wasserverschiebungen zwischen Zellinnerem und Extracellulärraum wurden nur durch Cocain beeinflußt.
Aufgrund der beobachteten Einwirkungen auf die bioelektrischen Membraneigenschaften und den Elektrolythaushalt der Zelle wird der erregbarkeitshemmende und leitungsblockierende Effekt der Lokalanaesthetica mit einer Behinderung des Na+-Einstromes in die Zelle bzw. einem Eingriff in den Mechanismus, der zu Beginn der Erregungsphase zu einem plötzlichen Anstieg der Na+-Permeabilität führt, erklärt. Es wird weiterhin gefolgert, daß die Lokalanaesthetica zwei verschiedene Membranfunktionen beeinflussen bzw. an zwei differenten Wirkorten in der Membran angreifen.
Summary
The use of a carefully standardised microelectrode technique coupled with the use of pairs of frog sartorius muscles has enabled small changes in the resting or action potential to be detected in drug-treated muscles. The local anaesthetics cocaine, procaine and lidocaine produce a progressive slow depolarization of the resting potential of up to about 5 mV per hour in concentrations of 10−4 to 10−3 g/ml. This process was completely reversible even after four hours and is dose sensitive. Cocaine was slightly more effective than procaine or lidocaine on a molar basis, and the smallest dose which usually produced an effect after four hours was 10−4 g/ml. No hyperpolarization at lower doses was detected.
Analysis of groups of pairs of muscles treated for four hours with equivalent doses of local anaesthetic showed either no significant change in ionic content or a significantly greater loss of potassium and gain of sodium in the control muscles. No significant difference was detected between treated and control groups for chloride content. These results are compatible with the idea that local anaesthetics interfere with the passive movement of potassium and sodium through the membrane and they also show that the large changes in resting potential are not due to changes in internal ionic concentration.
The action potential mechanism is affected in a matter of minutes in doses of the order of 10−3 g/ml, long before there is any detectable effect on the resting potential. There was no essential difference between the action of the three local anaesthetics. At concentration of about 10−4 g/ml excitability was present for about 1 hour with a raised threshold to stimulation. During this time the rate of rise and rate of fall of the action potential gradually decreased and the action potential magnitude steadily diminished, and the threshold increased about fourfold. The onset of complete block was markedly influenced by the rate of stimulation and the number of previous stimuli.
