Zusammenfassung
An insgesamt 20 erwachsenen nierengesunden und nierenkranken Personen wird unter standardisierten diätetischen und technischen Bedingungen der Einfluß von i.v. infundiertem synthetischem Bradykinin auf die Nierenfunktion geprüft. Die nierenwirksame Schwellendosis von Bradykinin liegt bei 0,07 μg/min und Kilogramm Körpergewicht. Nierengesunde und nierenkranke Personen reagieren auf Bradykinin qualitativ gleich.
Während i.v. Infusion von 0,15–0,30 μg synthetischem Bradykinin/min und Kilogramm Körpergewicht nehmen die renale Inulin- und p-Aminohippursäure-Clearance im Mittel um 19,3 (P<0,001) bzw. 13,2 (P<0,01) und die Filtrationsfraktion im Mittel um 6,5% (P < 0,05) der entsprechenden Ausgangswerte zu. Unter dem Einfluß von Bradykinin ist der (errechnete) renale Gesamtgefäßwiderstand infolge einer Verminderung der Teilwiderstände in den afferenten Arteriolen und in den Nierenvenolen (bei Abfall des mittleren arteriellen Blutdruckes um 5–10 mm Hg) erheblich herabgesetzt. Trotz einer gegenüber den Kontrollen um durchschnittlich 27,1 bzw. 20,1% (P<0,01 bzw.<0,05) gesteigerten renalen Eliminierung von Natrium und Kalium vermindern sich die Harnosmolarität und das osmotische Konzentrationsverhältnis von Urin zu Plasma (U/Posm) im Mittel um 22,2 bzw. 22,6% der Kontrollmittelwerte (P jeweils<0,001), während die Clearance von osmotisch freiem Wasser (CH 2 O) signifikant zunimmt. Ohne wesentliche Änderung der Serumosmolarität kommt es unter der Wirkung von Bradykinin zu einer mittleren Zunahme des Harnzeitvolumens um 56,2% (P<0,01) und der osmolaren Clearance (Cosm) um 25,8% (P<0,01).
Es wird vermutet, daß die diuretische Wirkung von Bradykinin durch eine Vergrößerung der Nierenmarkdurchblutung (Auswascheffekt des medullären osmotischen Konzentrationsgradienten) zustandekommt. Möglicherweise spielt Bradykinin (oder ein ähnlicher Stoff) als stärkster bisher bekannter biogener Vasodilatator bei der Regulierung der Nierenmarkdurchblutung, die keinen autoregulativen Mechanismen unterliegt, eine physiologische Rolle.
