Summary
Neuromyositis may be considered merely a complex of symptoms arising both from myositis and neuritis (neuropathy). Both the neuritic and myositic component can be recognized clinically or pathologically, or else, by applying both methods.
On grounds of published reports and 5 own cases the syndrome neuromyositis is divided into 5 groups with further subdivisions:

I.
In case of a typical dermato-polymyositis the involvement of peripheral nerves may be secondary. I/1. The inflammatory process extends from the muscle toward the nerve. I/2. The nerve fibres may be compressed by the inflammatory edema of the surrounding muscle. I/3. In a patient restricted in his motility because of myositis, a nerve lesion may arise from pressure paralysis. I/4. A dermatopolymyositis may be accompanied by polyneuropathy due to avitaminosis.

II.
In case of a dermato-polymyositis a polyneuritis unrelated to the primary disease may develop e.g. polyneuritis as a result of alcoholism, diabetes etc.

III.
Theoretically polymyositis and peripheral nerve lesions may occur simultaneously in any collagen disease. III/1. This is the case most frequently in polyarteritis nodosa; here the frequent ischemic polyneuropathy may eventually coincide with a myositis. III/2. According to some references a neuromyositic syndrome may also develop on the basis of a systemic lupus erythematosus. This, however, has yet to be confirmed by pathologic-anatomical evidence. III/3. The author reports the first case of neuromyositis of sclerodermic origin. III/4. In case of a primary chronic polyarthritis, neuromyositis may develop only if the interstitial myositis becomes associated with ischemic polyneuropathy on the basis of a panarteritis nodosa or rheumatic arteritis. III/5. In Sjögren's syndrome no case to be classified as neuromyositis has been published as yet.

IV.
The syndrome of neuromyositis is caused by the simultaneously appearing polymyositis and neuritis. IV/1. A known etiologic factor such as syphilis, may produce both myositis and neuritis. IV/2. Other known pathogenetic mechanisms, e.g. an allergy, may cause myositis and neuritis to develop concurrently. IV/3. In the presence of a primary dermatomyositis or polymyositis clinical and/or pathologic-anatomical methods will also reveal a neuritis. This group corresponds best to the concept of neuromyositis. The author's cases No. 3, 4 and 5 are listed under this group.

V.
In case of polyneuritis, inflammatory changes as concomitant phenomena may be present also in muscles.
The existence of neuromyositis, ardently debated in the pertinent literature, is being confirmed by the 5 cases presented by the author. Neuromyositis is to be considered a combination of myositis and neuritis (neuropathy) forming a complexity of syndroms, rather than a nosologic or clinico-pathologic-anatomical entity.
Zusammenfassung
Die Neuromyositis ist lediglich als ein Symptomenkomplex, zusammengesetzt aus Myositis und Neuritis (Neuropathie) anzusehen. Die neuritische und myositische Komponente können klinisch bzw. pathologisch oder auch unter Anwendung beider Methoden erkannt werden.
An Hand der vorliegenden Literaturberichte und fünf eigener Fälle wird das Neuromyositis-Syndrom in fünf Gruppen gegliedert, in denen weitere Untergruppen zu unterscheiden sind:

I.
Bei einer typischen Dermato-Polymyositis können die peripheren Nerven sekundär geschädigt werden. I/1. Der entzündliche Prozeß breitet sich vom Muskel auf den Nerven aus. I/2. Das entzündliche Ödem des Muskels kann die in ihm verlaufenden Nervenfasern komprimieren. I/3. Bei einem wegen Myositis bewegungsbeschränkten Kranken kann eine Nervenläsion durch Drucklähmung entstehen. I/4. Eine Polyneuropathie auf der Basis einer Avitaminose kann sich zu einer Dermato-Polymyositis hinzugesellen.

II.
Bei einer Dermato-Polymyositis kann eine von der Grundkrankheit unabhängige, z. B. alkoholische, diabetische usw. Polyneuritis in Erscheinung treten.

III.
Bei jeder Kollagenerkrankung können theoretisch Polymyositis und periphere Nervenläsion zu gleicher Zeit auftreten. III/1. Dies kommt am häufigsten bei der Polyarteriitis nodosa vor; hier kann die häufige ischämische Polyneuropathie mit einer eventuellen Myositis zusammentreffen. III/2. Nach einigen Literaturberichten kann sich auch auf der Grundlage eines systematischen Lupus erythematosus ein neuromyositisches Syndrom entwickeln. Dies bedarf jedoch noch einer pathologischanatomischen Bestätigung. III/3. Der Verfasser teilt den ersten Fall von Neuromyositis sklerodermischer Genese mit. III/4. Bei einer primären chronischen Polyarthritis kann eine Neuromyositis nur dann zustande kommen, wenn eine interstitielle Myositis sich mit einer ischämischen Polyneuropathie auf der Basis einer Panarteriitis nodosa oder einer rheumatischen Arteriitis vergesellschaftet. III/5. Beim Sjögren-Syndrom wurde bislang ein als Neuromyositis zu klassifizierender Fall nicht mitgeteilt.

IV.
Das Syndrom der Neuromyositis wird durch das nebeneinander bestehende Auftreten von Polymyositis und Neuritis hervorgerufen. IV/1. Ein bekannter ätiologischer Faktor, z. B. die Syphilis, kann gleichzeitig Myositis und Neuritis verursachen. IV/2. Auf der Basis irgendeines bekannten pathogenetischen Mechanismus, z. B. eine Allergie, entstehen gleichzeitig Myositis und Neuritis. IV/3. Bei Vorliegen einer primären Dermatomyositis oder Polymyositis kann mittels klinischer und/oder pathologisch-anatomischer Methoden auch eine Neuritis nachgewiesen werden. Diese Gruppe entspricht am ehesten dem Begriff der Neuromyositis. Die Fälle 3, 4 und 5 des Verfassers werden in diese Gruppe eingeordnet.

V.
Bei einer Polyneuritis können entzündliche Veränderungen als Begleiterscheinung auch in der Muskulatur vorhanden sein.
Die in der Literatur umstrittene Existenz der Neuromyositis wird durch die fünf Fälle des Verfassers bekräftigt. Die Neuromyositis kann nicht als eine nosologische oder klinisch-pathologisch-anatomische Einheit, sondern lediglich als eine Kombination von Myositis und Neuritis (Neuropathie) zu einem Symptomenkomplex betrachtet werden.
