Zusammenfassung
1. Die Zweckmäßigkeit der Carotisligatur in der Behandlung sackförmiger Hirnaneurysmen wird in Frage gestellt.
2. Seit einigen Jahren ist systematisch eine Freilegung des Aneurysmas mit der Absicht, die Ansatzstelle des Aneurysmas an der Hauptarterie zu unterbinden, vorgenommen worden. Dies ist in 60 von 78 Fällen gelungen.
3. Für eine erfolgreiche Operation ist die künstliche Blutdruckherabsetzung durch ganglienblockierende Mittel von ausschlaggebender Bedeutung.
4. Im akuten Blutungsstadium mit Bewußtlosigkeit wird jetzt eine Operation abgelehnt.
5. Im subakuten Blutungsstadium wird auf die Bedeutung des Auftretens eines Gefäßspasmuses für das klinische Bild und die Operationsindikation hingewiesen.
6. Im freien Intervall ist die operative Freilegung des Aneurysmas mit Stielligatur heutzutage als eine sichere Methode anzusehen.
