Systematische Befunddokumentation bei der Retinopathia diabetica

Zusammenfassung

Es wird auf die Bedeutung einer exakten Befunddokumentation bei der Retinopathiadiabetica - besonders bei der Verlaufsbeobachtung zur Beurteilung einer Behandlung — hingewiesen. Zu bisherigen Verfahren wird kritisch Stellung ge nommen. Für die Befunddokumentation wird die spezielle Anordnung einer Photomontage empfohlen. Zur Auswertung solcher Bilder wird ein Verfahren demonstriert, das eine zahlenmäßige Kontrolle von roten Punkten (Mikroaneurysmen und Blutungen), Exsudaten und Gefäßproliferationen gestattet.

Résumé

L'importance d'une documentation objective de la rétinopathie diabétique est évidente. Les descriptions et les dessins de la rétine diabétique sont très variables et de plus très discutables. La photographie du fond d'oeil donne une documentation objective. Nous proposons une photographie qui se compose de 15 photos isolées; cette photographie de mosaïque montre 70 degrés du pôle postérieur qui est divisé en 12 secteurs égaux. Nous comptons les points rouges (hémorrhagies et micro-anévrysmes) et les points blancs (exsudats mous et durs). Les néo-vaisseaux sont mesurés par l'absence ou la présence dans un secteur.

Summary

The importance of a systematic and objective documentation of the fundus findings in diabetic retinopathy is demonstrated.

There are several possibilities to register diabetic changes, for instance to describe the findings in the records or to draw very „naturalistic” pictures. The objective way to record diabetic retinopathy is by photography. It depends on the interesting problem whether the use of single pictures or of a photo-assembly (panorama-picture) ist better. For follow-up studies the panorama-picture gives the better survey.

The expense of work for this kind of documentation is only justified if a biometric evaluation of the data is done too. A scheme is presented in which the panorama-picture (about 15 single-photographies = about 70 degree of the posterior pole) is divided in sectors of equal plain. In these sectors the diabetic red spots and exsudates can be counted without differentiation between microaneurysms and haemorrhages. Only greater spots (like a quarter of the disc) with irregular limitation are likely haemorrhages and can be counted separately. Also it is difficult to distinguish between soft and hard exsudates especially in the fundus periphery because of the variable quality of the pictures. Vascular and tissue proliferation in the different sectors will be recorded by means of present or not present. With these data a statistical follow-up is planned in natural course and after treatment of diabetic retinopathy.
