Unusual lesions in the intima and endocardium caused by a suicide carried out by infusion of thiobutabarbital

Summary

A suicide is reported that was carried out by i.v. infusion of thiobutabarbital. The most impressive findings were macroscopical alterations in the venous endothelium, the endocardium of the right atrium and ventricle, and the endothelium of the pulmonary artery. The same local cytotoxic side effects could be reproduced in animal experiments. The lesions are most striking after circulatory stagnation in the agonal and supravital period when highly concentrated thiobarbiturates come into contact with the endothelium.

Zusammenfassung

Nach i.v.-Infusion von Thiobutabarbital bei einem Suizidanten ließen sich bereits makroskopisch auffällige Endothelschäden im Verlaufe der zum Herzen führenden Venen, am Endokard des rechten Vorhofs und Ventrikels so wie am Endothel der A. pulmonalis beobachten. Gleichartige Endothelläsionen konnten im Tierexperiment reproduziert werden. Es handelt sich um eine lokale endothel-toxische Wirkung dieses Thiobarbiturats. Die Läsionen sind besonders ausgeprägt, wenn agonal und in der Supravitalphase bei sistierendem Blutstrom das Barbiturat in relativ hoher Konzentration in intensiven Kontakt mit der Gefäßwand gelangt.
