Prognostic factors for bone metastases in breast cancer

Summary

Two hundred and sixty-nine patients with breast cancer were assessed with regard to multiple parameters at the time of initial diagnosis. Over a follow-up period of 6 or 7 years, 84 patients developed skeletal metastases and in 42 they were symptomatic. Symptomatic bone metastases were related to lytic metastases and an elevated sedimentation rate. The significant parameters predisposing to the development of bone metastases were the presence of adenopathies or metastases at the time of initial diagnosis. The number of axillary nodes involved was related to the number of bone metastases, the number of spontaneous abortions, and the gonadal endocrine function. The adherence of axillary nodes was related to the number of bone metastases, an elevated sedimentation rate and the serum calcium level.

Résumé

Un groupe de 269 patientes atteintes de cancer du sein a été étudié afin de chercher quels facteurs permettraient de prévoir l'apparition de métastases osseuses. Les seuls paramètres significatifs sont les classes N et M de la classification TNM. Le type de métastase et la vitesse de sédimentation (VS) sont en relation avec l'apparition des symptômes cliniques. On a étudié deux facteurs pronostiques connus: l'importance et la mobilité des adénopathies axillaires. Le premier, le nombre de ganglions (±3 ou >3) est correlé avec la classification TNM, le nombre de métastases osseuses, le taux sérique de LDH, le nombre d'avortements spontanés et la fonction gonadique endocrine. Le second, l'adhérence des ganglions, est correlé avec les classes T et M, le nombre de métastases, la VS et la calcémie.
