Evaluation of joint thickness in the hip before and after osteotomy using three-dimensional modelling

Summary

Joint thickness was assessed using three-dimensional modelling techniques before and after the Bombelli intertrochanteric valgus-extension osteotomy. Before operation, the average thickness of the entire hemispherical surface was 2.3 mm, which was significantly thinner than that of the controls. After operation, 11 of 12 patients showed an average increase in thickness to 3 mm, nearly equal to that of the controls, and this was accompanied by clinical improvement.

Résumé

L'épaisseur de l'interligne articulaire de la coxo-fémorale a été évaluée par une technique de modélisation tri-dimentionnelle avant et après l'ostéotomie de Bombelli (ostéotomie intertrochantérienne de valgisation-extension). Avant l'intervention, l'épaisseur moyenne de l'interligne sur toute la surface hémisphérique était de 2,3 mm (n=12), ce qui est significativement inférieur à celle des témoins (p<0,001). Après l'opération 11 cas sur 12 ont montré une augmentation de l'épaisseur avec une moyenne qui atteint 3 mm (p<0,001). L'épaisseur de l'interligne des patients ayant été opérés (n=12) était pratiquement égale à celle des témoins. Cette épaisseur s'est accrue sur toute l'étendue de l'articulation et cela s'est accompagné d'une amélioration du score clinique de Harris.
