Problems concerning diagnosis and surgical treatment of choledochal cysts: a case report

Abstract

We report a case of choledochal cyst accompanied by an adenoid gallbladder carcinoma, in an adult patient. Diagnosis was complicated by coexisting primary hyperparathyroidism. In view of the pathological laboratory tests, which suggested acute pancreatitis, the visualized cystic abdominal mass was regarded as a pancreatic pseudocyst. Neither ultrasound nor CT could clearly define the cyst's origin. Laparotomy, performed later because of deteriorating clinical condition, showed an extended carcinoma which was already invading the liver. Only palliative resection and choledochocystojejunostomy was technically feasible. To prevent the development of malignancies in dilated bile ducts, early treatment by cyst resection and reconstruction of the extrahepatic biliary system is recommended, especially in younger patients.

Zusammenfassung

Wir berichten über den Fall einer Patientin, bei der im Zusammenhang mit einer Choledochuszyste ein adenoides Gallenblasenkarzinom entstanden war. Die Diagnosefindung war durch das gleichzeitige Vorliegen eines primären Hyperparathyreoidismus erschwert, da aufgrund der pathologischen Laborergebnisse primär der Verdacht auf eine akute Pankreatitis bestand. Die nachgewiesene abdominale zystische Raumforderung war zunächst — in Sonographie und CT — als Pankreaspseudozyste fehlgedeutet worden. Die dann wegen einer zunehmenden klinischen Verschlechterung durchgeführte Laparotomie zeigte einen fortgeschrittenen Karzinombefund, bereits mit Infiltration der Leber. Es erfolgte eine Palliative Resektion und Choledochozystojejunostomie. Um einer malignen Entartung in dilatierten Gallengängen vorzubeugen, ist besonders bei der Behandlung von Kindern und Jugendlichen eine frühzeitige Resektion und Rekonstruktion des extrahepatischen biliären Systems empfohlen.
