Summary
Congenital dislocation of the knee (CDK), first described in 1922, is a rare condition sometimes associated with other congenital malformations. There is a total anterior dislocation with recurvatum and upward displacement of the proximal tibia. This review covers our ten year experience at the hospital San Juan de Dios, in Caracas, Venezuela, a children's hospital with three orthopaedic services. We have seen 22 affected knees in 14 patients. Newborn patients were treated on traction until reduction was achieved, followed by serial plaster of Paris casts until, optimally 90° of flexion was gained. Surgical treatment was used if conservative treatment had failed after six months, or if the patient first presented after the age of 12 months. All our patients were eventually able to walk, three with the assistance of calipers. Some limitation of knee flexion was commonly seen. The best results were achieved when treatment was started early.

La luxation congénitale du genou (L.C.G.), décrite en 1922, est une affection rare quelquefois associée à d'autres malformations congénitales. Elle consiste en un déplacement antérieur avec recurvatum et un déplacement vers le haut de l'extrémité supérieure du tibia. En 10 ans, à l'hôpital San Juan de Dios à Caracas, Vénézuela, nous en avons observé 14 cas, avec un total de 22 genoux. Nous utilisons la traction au zénith, chez les nouveaux-né jusqu'à obtenir une réduction, puis une série de plâtres est utilisée jusqu'à obtenir 90° de flexion (trois patients). Un traitement chirurgical est indiqué après l'âge de 6 mois dans les cas oòu le traitement conservateur n'a pas donné de résultats satisfaisants (six patients) ou si l'enfant a été examiné après l'âge d'un an (cinq patients). Tous les patients ont été capables de marcher; trois ont eu besoin d'orthèses. La limitation des derniers degrés de flexion du genou est très courante. Plus précocement le traitement est entrepris, meilleurs sont les résultats.
