Summary
Olecranisation of the patella was described and first used by Grammont (1984) to maintain reduction of the knee joint after posterior cruciate repair and reconstruction. Since 1985 we used this technique in 18 acute posterior ruptures and knee dislocations which did not undergo surgery. Knee laxity is assessed under general anaesthesia with radiographic control. A 4 or 5 mm. Steinmann pin is introduced medially at the top of the patella and drilled vertically through the bone to continue behind the patella tendon. After reduction of the posterior drawer, the pin is passed into the anterior part of the tibia. Full mobility between 0° and 90° is maintained. Physiotherapy started immediately and early weight bearing is allowed with a posterior splint. Olecranisation prevents posterior subluxation of the tibia and gives an anterior tibial subluxation force which is minor in flexion but major in extension. We followed up our patients for 1 to 8 years. The latest testing with radiographs demonstrate posterior drawer but all patients report good results. Early physiotherapy avoids stiffness, amyotrophy, and reflex sympathetic dystrophy. Olecranisation appears to give similar results more quickly and with fewer complications than P. C. L. surgery.
Résumé
L'olécranisation de la rotule a été décrite et utilisée par Grammont pour empêcher le tiroir postérieur après réparation chirurgicale du L. C. P. Depuis 1985 nous avons utilisé cette technique dans les entorses fraîches du L. C. P., et dans les luxations du genou, sans faire de réparation ligamentaire. Après un testing avec radiographies permettant de connaître exactement l'importance des lésions ligamentaires centrales et périphériques, une broche de Steinmann est introduite au moteur au sommet de la rotule. Elle la traverse parallèlement à sa surface articulaire, et sort en arrière de la pointe de la rotule, puis passe en arrière du tendon rotulien. Après réduction du tiroir postérieur, la broche est introduite dans le tibia par la surface pré-spinale. La mobilité est testée entre 0° et 90°. La rééducation du genou est commencée immédiatement. L'appui est autorisé sous couvert d'une attelle postérieure. Notre étude biomécanique montre que l'olécranisation empêche la subluxation postérieure du tibia et qu'elle peut créer une force de subluxation antérieure du tibia maximum en extension, minimum en flexion. Un tiroir postérieur, le plus souvent modéré, est constamment retrouvé dans nos 18 cas, il a été suivi par des mesures radiologiques avec un recul de 1 à 8 ans. Les résultats de notre série montrent une excellente tolérance fonctionnelle. La rééducation précoce évite raideur et amyotrophie voire algodystrophie. Par rapport au traitement chirurgical, les résultats sont obtenus plus rapidement avec moins de complications et semblent aussi bons sur la laxité résiduelle.
