Anterior spinal artery syndrome

Summary

Three patients with symptoms due to the anterior spinal artery syndrome were treated by direct perfusion of dexamethasone sodium phosphate and urokinase into the artery of Adamkiewicz. Their symptoms were paraparesis with dissociated sensory loss and sphincter dysfunction, and there was no evidence of the possible cause. In the early phase of the disease, three consecutive injections were carried out with an interval of a week between each. All the patients made a good recovery.

Résumé

Trois patients présentant une paraparésie due à une insuffisance de l'artère spinale antérieure ont été traités par perfusion directe de phosphate sodique de dexaméthasone et d'urokinase dans l'artère d'Adamkiewicz. Le tableau clinique comportait une paraparésie avec perte dissociée de la sensibilité et troubles sphinctériens, sans cause évidente. A la phase initiale de la maladie, trois essais consécutifs d'injection de stéroïdes et d'urokinase dans l'artère ont été réalisés à une semaine d'intervalle. Tous les malades ont récupéré. On peut proposer l'injection de stéroïdes et d'urokinase directement dans l'artère d'Adamkiewicz pour obtenir l'amélioration éventuelle d'une fonction médullaire compromise.
