Abstract

A 4-year-old child with traumatic rupture of the entire right hemi-diaphragm and herniation of the liver into the chest is described. The injury is uncommon and the diagnosis by ultrasonography, fluoroscopy, gastrointestinal contrast study, and CT scanning may be difficult or misleading. A non-ionic contrast, Omnipaque 240 (Iohexol) instilled via the intercostal catheter provides a definitive diagnosis rapidly and without complication.
