Zusammenfassung
An Hand eines Falles von einem benignen Melanocytom der Papille werden die klinischen und histopathologischen Eigenschaften dieser Tumorart herausge-stellt: besonders kennzeichnend ist das Fehlen subjektiver Sehstörungen und die zufällige Entdeckung eines dunkelbraun bis schwarzen kleinen Tumors, der exzentrisch dem Sehnervenkopf aufsitzt. Die Sehschärfe bleibt fast immer normal, es läßt sich lediglich eine Vergrößerung des blinden Flecks nachweisen. Die Geschwulst, die aus kräftig pigmentierten, harmlos aussehenden Zellen besteht, wächst entweder gar nicht oder nur sehr langsam. Die Differentialdiagnose gegenüber anderen pigmentierten Papillentumoren wird erördert. Von einer sofortigen Enukleation der betroffenen Augen ist abzuraten und statt dessen eine sorgsame klinische Kontrolle des ophthalmoskopischen Bildes und der Funktion zu empfehlen.
Résumé
Les propriétés cliniques et histopathologiques d'un melanocytome bénin de la papille sont présentées.
L'absence de troubles subjectifs visuels est particulièrement caractéristique ainsi que la découverte fortuite d'une petite tumeur variant du brun foncé au noir et située excentriquement par rapport à la papille. L'acuité visuelle demeure presque toujours normale; on démontre occasionellement un agrandissement de la tâche aveugle. La tumeur composée de cellules d'apparence bénigne fortement pigmentées se développe soit très lentement soit pas du tout.
Le diagnostic différentiel par rapport aux autres tumeurs de la papille est discuté.
L'enucléation immédiate de l'oeil atteint est déconseillée et l'on recommande à la place un contrôle clinique vigilant de l'image ophtalmoscopique et du fonctionnement de l'oeil.
Summary
A case of benign melanocytoma of the optic nerve head is reported and the clinical and histopathological characteristics of the tumor are described. Characteristically there are no subjective complaints and the tumor is usually discovered incidentally as a small black lesion sitting eccentric on the disc. Visual acuity remains normal and only a slight enlargement of the blind spot is noted.
The tumor is composed of heavily pigmented harmless appearing cells and progresses very slowly or not at all. The differential diagnosis also of other pigmented disc tumors is discussed.
Instead of immediate enucleation pigmented tumors of the optic disc without marked functional defects deserve careful and close observation of the ophthalmoscopic findings and the visual field.
