Granulosa Cell Tumor of the Ovary in a Benign-looking Adnexal Mass: A Rare Occurrence and its Management

Abstract

Introduction

Granulosa cell tumors (GCT) are rare tumors which account for 1–2 % of all ovarian malignancies and juvenile type of GCT affecting young women are even rarer comprising only 5 % of all GCT’S. Laparoscopy despite increasingly being adopted for the management of all gynecological diseases, still has a controversial role in surgical staging and management of early stage ovarian cancer, major concerns being accuracy of surgical staging, tumor spillage and port site metastasis.

Case Report

We hereby report an extremely rare case of granulosa cell tumor in an unmarried girl who presented with complaints of dysmenorrhoea. Ultrasound scan showed a simple ovarian cyst of 8 × 9 cm in right adnexa and all the relevant tumor markers were negative. An uneventful laparoscopic ovarian cystectomy was performed but to our utter surprise, histopathological examination revealed granulose cell tumor.

Discussion

Rate of unexpected ovarian cancer in preoperatively benign looking adnexal masses is 0.38–18.67 %. Till date, only one case of unexpected GCT is reported in a laparoscopically operated adnexal mass.

Conclusion

The authors present a case of this rare tumor discussing the pitfalls of diagnosis, while looking at the management options.
