Samenvatting
Een 13-jarig meisje presenteert zich op de spoedeisende hulp met dyspneu, pijn op de borst en koorts. Initieel wordt gedacht aan een pericarditis, maar bij verdere evaluatie blijkt er sprake te zijn van een pneumomediastinum. Er wordt een conservatief beleid gevoerd, waarop het meisje vanzelf opknapt en naar huis kan worden ontslagen. Het spontaan pneumomediastinum ontstaat doordat enkele alveoli kapotgaan en de lucht die hieruit vrijkomt door de borstholte migreert naar het mediastinum. Het is een zeldzame, maar benigne en meestal self-limiting aandoening die bij kinderen meestal veroorzaakt wordt door astma. De belangrijkste klachten zijn dyspneu, pijn op de borst en subcutaan emfyseem, de diagnose wordt bevestigd door middel van een thoraxfoto. Bij een kind met dyspneu en/of pijn op de borst moet het spontaan pneumomediastinum in de differentiaaldiagnose worden opgenomen.
Summary
A 13-year-old girl presents to the emergency department with dyspnea, chest pain and fever. Initially, pericarditis is considered likely, but after further evaluation she is diagnosed with a pneumomediastinum. She is treated conservatively, which leads to spontaneous recovery and eventually discharge. A spontaneous pneumomediastinum is caused by damage to alveoli, allowing passage of air from the lungs into the mediastinum. It is a rare but benign and almost always self-limiting disease. In children, asthma is the major cause. The characteristic symptoms or signs are dyspnea, chest pain and subcutaneous emphysema, and the diagnosis is confirmed by a positive chest X-ray. In the case of a child with dyspnea or chest pain a spontaneous pneumomediastinum must be considered as a possible diagnosis.
