Samenvatting

Achtergrond

De sterke heterogeniteit van autismespectrumstoornissen (ASS) compliceert het klinisch en wetenschappelijk werk.

Doel

In deze studie worden homogenere ASS-subgroepen gemodelleerd op basis van vroege trajecten van autismesymptomen. Traject-subgroepen werden verder gekarakteriseerd aan de hand van het beloop van taal, cognitie, bijkomende (ADHD-gerelateerde) gedragsproblemen en klinische uitkomstdiagnose.

Methode

Kinderen (N = 203), in de leeftijd van 1 tot 4 jaar en verwezen op verdenking van ASS, werden drie keer onderzocht met intervallen tussen 9 maanden en 3 jaar. De onderzoeken omvatten gestandaardiseerde maten voor ASS, taal, non-verbaal IQ en door ouders gerapporteerde gedragsproblemen (ITSEA, CBCL).

Resultaten

Met behulp van een latente-klasse-groeicurve-analyse met ADOS-totaalscores konden drie stabiele hoofdgroepen worden geïdentificeerd en twee kleine verbeterende groepen.

Conclusies

De resultaten bevestigen de sterke stabiliteit van autismesymptomen en benadrukken tevens dat de vroege trajecten van taal, non-verbale cognitie en ADHD-gerelateerde kenmerken bijdragen aan het definiëren en verklaren van de verschillende ontwikkelingstrajecten van ASS.

Variation in the early progression of autism symptoms

Abstract

Background

The high heterogeneity of autism spectrum disorders (ASD) across multiple domains complicates research and clinical work.

Objective

To model more homogeneous subgroups within ASD based on early progress of core symptoms and further characterize these subgroups in terms of progression of language, cognition, co-occurring (ADHD-related) traits and clinical outcome diagnosis.

Methods

Children (n = 203) referred for possible ASD at age 1–4 years were assessed at three points in time at intervals ranging from 9 months to 3 years. Assessments included standardized measures for ASD (ADOS), language (ADOS language item), non-verbal IQ (NV-IQ; various tests appropriate for the chronological/mental age) and parent-reported behavioural problems (ITSEA, CBCL).

Results

Latent-class growth curve analysis with ADOS overall scores led to identification of three main stable and two small improving groups.

Conclusions

Results support the high stability of ASD symptoms, but also highlight a significant role for non-ASD domains in defining and explaining the various ASD progression paths.
