Hoofd-halscarcinomen zijn, met 3% van de maligniteiten, in Nederland tamelijk zeldzaam; ze komen bij mannen vaker voor dan bij vrouwen. Roken, alcoholgebruik en irriterend voedsel (betelnoten) zijn, zeker in combinatie, bekende risicofactoren. Een andere risicofactor is het humaan papillomavirus (typen HPV16 en HPV18), dat verantwoordelijk is voor een toename in het aantal orofarynxcarcinomen. Hoofd-halstumoren komen vaak pas laat aan het licht, omdat de eerste klachten meestal aspecifiek zijn en de tumor met lichamelijk onderzoek niet altijd eenvoudig kan worden gevonden. De kno-arts stelt de diagnose op basis van endoscopische inspectie en biopten.
Toch is de huisarts een onontbeerlijke schakel in de preventie, signalering en behandeling van deze maligniteiten. Ten eerste is zo vroeg mogelijke verwijzing erg belangrijk. Alertheid is geboden bij patiënten met aanhoudende klachten in het hoofd-halsgebied, zeker als deze eenzijdig zijn of optreden in combinatie met de genoemde risicofactoren. Ten tweede zijn de ziekte zelf en de noodzakelijke chirurgie en radiotherapie buitengewoon ingrijpend voor het functioneren (ademen, eten, communiceren) en het uiterlijk van de patiënt. Tijdens de follow-up en de multidisciplinaire revalidatiebehandelingen speelt de huisarts een grote rol in de psychosociale begeleiding van patiënten.

Cancer of the head and neck is relatively rare in the Netherlands, accounting for 3% of all malignancies. It occurs more often in men than in women, and smoking, alcohol consumption, and irritating food (such as betel nuts), especially in combination, are recognized risk factors. Another risk factor is human papilloma virus (types HPV16 and HPV18), which is responsible for an increase in oropharyngeal cancer. Cancer of the head and neck is often detected late because the first symptoms are often not specific and the tumour is not detected during physical examination. Although the diagnosis is made by ENT doctors on the basis of endoscopy and biopsy findings, general practitioners have an essential role in the prevention, detection, and treatment of these cancers. In the first instance, early referral is important. Doctors should be alert to persistent symptoms in the head and neck area, especially if these occur on one side only or in combination with the above-mentioned risk factors. In the second instance, the surgery and radiotherapy needed to treatment these cancers have an enormous impact on the functioning (breathing, eating, communication) and appearance of patients. General practitioners have an important role in the psychosocial management of patients during their multidisciplinary treatment and follow-up.
